Description
This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued The proposed contract action is for supplies or services for which the Government intends to solicit competitive offers. This requirement is set aside for small business. Interested persons may submit offers, and all eligible offers received prior to September 16, 2011 will be considered by the Government. The NAICS code is 334119 and the size standard is 500 Employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53. The system shall be web-based and operates on its own server's independent from the vendor or the VA's VistA. Care data are inputted at the point of care using wall-mounted touch-screen kiosks. Care data shall be accessed from any PC on the network and used to manage frontline staff by messaging to the kiosks and data crunching. The implementation shall include all required hardware, software, licensing, training, and ongoing advanced support as well as seamless integration with MDS 3.0. The system shall use 135 wall-mounted touch screen kiosks throughout the VISN to allow staff members to quickly and easily record supportive documentation of all types. Information for all departments shall be easily retrievable in a variety of formats for MDS completion, care planning, and staff management. VISN 21 requires installation, set-up, staff training, and ongoing support. The initial set-up shall include on-site training of VA staff and follow-up training after the first six months of operation. The ongoing support package shall include premium services for software maintenance, troubleshooting, repairs and updates on a 24/7 basis. 1.Kiosks. 135 wall-mounted touch screen kiosks with a 3 year warranty. The kiosks shall run a version of Windows XP and shall be capable of running antivirus and other protection software. The data shall be transmitted over the existing network and inputted through hardwire connections. The kiosks shall have wireless capabilities and handheld mobile input devices shall also be available for future applications. The bandwidth required for the kiosks shall not significantly affect existing data transmission rates. 2.Servers. Two dedicated servers for the system. One server shall run the applications and the second server the database. 3.Care Tracking Module. The care tracking module provides the primary functionality of the system, including resident/staff maintenance, assessment/series creation and customization, scheduling, help and tutorials, reports, kiosk alert reporting module, more than 100 pre-established management reports, enterprise-wide reporting, role-based access/security, and unlimited access to the management software from browser based workstations. System shall come with all applicable user licenses (i.e. one facility license for each facility using the system). 4.Charge Tracking Module. The charge tracker module provides additional primary functionality of the system, including resident/staff maintenance, item file maintenance, purchasing, restocking and par level adjustment features. The module shall come with all applicable user licenses (i.e. one facility license for each facility using the system). 5.Staff Messaging Module. The staff messaging module uses the kiosk as a way of communicating messages to the floor staff about specific resident conditions, general staff announcements, and tasks that need to be completed. Additionally, this allows messages to be sent to managers from the floor based upon answers to questions given on the kiosks. 6.Resident-Centered Programs Module. The resident-centered programs module tracks specific resident programs and goals such as documentation, including activities, restorative, toileting, turning and repositioning, and dietary, among others. "Proven interface compatibility at another existing VA facility. Equal products shall provide seamless interface with currently installed base Care Tracker equipment at the San Francisco VAMC. If equipment/software is not completely compatible, then the vendor would also have to quote replacement of currently installed base of equipment/software at the San Francisco site along with installation and training for San Francisco. EVALUATION FOR AWARD Offerors will be evaluated to the extent they can provide the items (or equal) listed in the price schedule in Section B. The Government will award to the lowest priced, responsible, technically acceptable offeror. The following will be considered in determining if the item offered is an acceptable "equal." a.
