- RFQ This is a combined synopsis solicitation to provide for a Cross-sectional survey to detect HIV prevalence and associated risk factors among male clients of sex workers in Hanoi and Ho Chi Minh City, Vietnam. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request for Proposal (RFP). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-53. The North American Industry Classification (NAICS) Code is 541611. The acquisition is being conducted on a Full and Open, Competitive Basis, using simplified acquisition procedures, FAR Part 13. A Firm Fixed Price purchase order is anticipated. Award shall be based on the oferors technical understanding of the requirement and the reasonableness of the proposed price. FAR Part 15.002(b), competitive procedures will be utilized. The proposal is due on September 18, 2011 at 1:00 p.m., Eastern Daylight Savings Time. All proposals are to be submitted electronically to Pellumbeshe Hoxhaj @ kfx2@cdc.gov. Please provide any questions in writing electronically to the aforementioned e-mail address by September 12, 2011 at 1:00 p.m. Eastern Daylight Savings Time. No telephone calls concerning questions regarding the RFQ will be accepted. In order to receive an award, a contractor must be registered in the Central Contractor Registration (CCR) data base. Proposals shall be written in English and separated into a Technical Proposal and a Business Proposal. The Technical Proposal shuld summarize the contractors approach to provide the services required in the PWS. It should address the Technical Approach, Key Personnel and Experience. The business Proposal should include: • Completed blocks 13.14, 15 and 16 of Standard Form SF18, the Request for Quotation (RFQ) • Completed Section II, Price List, of the RFQ • A breakdown of each Contract Line Item Number (CLIN) price as follows: Labor Category: Rate: Total Hours : Total Price: The offeror must have experience in conducting epidemiological studies in Vietnam supporting Vietnam and/or global HIV/AIDS activities and programs, as well as be able to put together a team of at least 5 investigators (including the principal investigator) to support this project. The principal investigator should hold an MD degree or doctorate in epidemiology, statistics, public health, social science research or related field and have a minimum of 10 years of public health research experience and 5 years of experience working or partnering with the Vietnam Ministry of Health to conduct epidemiologic studies. All investigators should be fluent in Vietnamese. Investigator's experience in HIV/AIDS study design and implementation is preferred. Selection for award shall be made based on "best value" after an assessment of the Offeror's proposals. The best value determination identifies the offer that represents the most advantageous and best value to the Government once all the factors are considered. Cost will not be weighted but the importance of cost will increase should Offerors be determined to be relatively equal in standing with other Offerors. The following Evaluation Criterias will be considered: 1. Technical approach: Provide a detailed description of understanding of the requirements specified in the Performance Work Statement (PWS), explain the proposed resources and approach for accomplishing the requirements identified in the PWS. The narrative shall also identify key milestones for accomplishing the work and ensuring timely delivery according to the delivery schedule and period of performance stipulated in the PWS 2.
