Description
Past Performance Questionnaire NAICS Code: 813110 Religious Organizations, this is a combined synopsis/solicitation for a commercial service prepared in accordance with the format set forth in Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This solicitation is for a base year and four (4) option years. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ); solicitation number FA7000-11-T-0100, in accordance with FAR parts 12 & 13. AVAILABILITY OF FUNDS. This is currently an UNFUNDED requirement and award is contingent upon appropriated funds being made available. The government bares no legal liability for bid cost associated with this request for quote. Vendors should not submit a quote unless they are willing to absorb all cost associated with submitting a quote. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-53, dated 4 August 2011. The North American Industrial Classification System (NAICS) number is 813110 and the business size standard is $7 Million. The proposed acquisition business size is 100% small business set aside. To assure timely and equitable evaluation of the offers, the offeror must follow the instructions contained herein. The quotes/responses must be complete, self-sufficient, and respond directly to the requirements of this solicitation. To be eligible for award the offeror's response to the RFQ must include the following: PRICE and PAST PERFORMANCE INFORMATION as described below. All responses to the RFQ must also include enough information to demonstrate the offer meets all requirements of the solicitation to be considered for award. b. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price. 1. PRICE INFORMATION Insert proposed UNIT and EXTENDED prices for all Contract Line Items (CLINS) listed above. The extended amount must equal the whole dollar unit price multiplied by the number of units. Include any pertinent information regarding industry standards that may be applicable. 2. PAST PERFORMANCE INFORMATION Provide a list of no more than three (3) references of the most relevant contracts performed for commercial customers, Federal agencies, or other military organizations that have used, or are currently using the same or similar service proposed for this contract. Provide only references for same or similar services or contracts. The government will evaluate the quality and extent of an offeror's performance deemed relevant to the requirements of this solicitation. A past performance survey is attached to this solicitation. It is the contractor's responsibility to forward the survey to the references for completion. Relevancy is defined as performance of contracts for same or similar work that are also similar or greater in scope, magnitude and complexity than the effort described in this solicitation. The evaluation of past performance information will take into account past performance information regarding key personnel who have relevant experience or subcontractors that will perform major critical aspects of the requirement when such information is relevant to the instant acquisition. Completed performance surveys are not to be reviewed by the offeror. ONCE THE SURVEY IS COMPLETED, IT IS CONSIDERED FOR OFFICIAL USE ONLY, and is only reviewed by Government personnel directly involved in the evaluation process for this acquisition. The references should be companies who have worked with the offeror within the past three years. It is the contractor's responsibility to ensure their references submit timely questionnaires in accordance with the solicitation. Alternate POC is Russell Jordan, Contracting Officer, (719) 333-3961, russ.jordan@usafa.af.mil. 52.212-2 EVALUATION-COMMERCIAL ITEMS (JAN 1999) ADDENDUM TO 52.212-2 -- Evaluation -- Commercial Items (Jan 1999). a. BASIS FOR CONTRACT AWARD: The Government intends to award contract to offeror which represents best overall value in accordance with FAR Parts 12 and 13 in which competing offerors' past performance history and price will be considered. By submission of its offer, the offeror accedes to ALL solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. The government reserves the right to reject any offer that does not meet all requirements of the solicitation. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. b. The evaluation will be conducted using simplified procedures outlined in FAR 13.106. In accordance with simplified procedures, contracting offices may conduct comparative evaluations of offers. Evaluation of other factors, such as past performance -- (i) Does not require the creation or existence of a formal data base; and (ii) May be based on one or more of the following: (A) The contracting officer's knowledge of and previous experience with the supply or service being acquired; (B) Customer surveys, and past performance questionnaire replies; (C) The Government wide Past Performance Information Retrieval System (PPIRS) at www.ppirs.gov ; or (D) Any other reasonable basis. c. The evaluation process shall proceed as follows: Step 1. Rank all proposals by price. IAW FAR 13.106-2 The contracting officer is allowed to make comparative analysis of offers to establish best overall value to the government. Step 2 - Evaluate all quotes for ability to meet all requirements of the solicitation. Offerors that do not conform fully to the solicitation will not be considered. Step 3 - Review any and /or all past performance information as stated above. d. PAST PERFORMANCE RATINGS: The performance assessment process will result in an overall performance rating of "Sufficient Past Performance" or "Limited Past Performance". Offerors with no relevant or recent past or present performance history shall receive the rating "Unknown Past Performance", meaning the rating is treated neither favorably nor unfavorably. Confidence ratings are defined as follows: SUFFICIENT PAST PERFORMANCE: Based on the offeror's performance record, the government has a high expectation that the offeror will successfully perform the required effort. LIMITED PAST PERFORMANCE: Based on the offeror's performance record, the government has a low expectation that the offeror will successfully perform the required effort. UNKNOWN PAST PERFORMANCE: Based on the offeror's performance record, the government has no expectation that the offeror will be able to successfully perform the required effort. e. When determining relevancy and assigning a past performance confidence rating, the government will consider the type of experience, the amount of experience, and the flexibility of contractor to meet customer needs. Past performance regarding predecessor companies, key personnel who have relevant experience or sub-contractors that will perform major or critical aspects of the requirement will not be considered as highly as past performance information from the principal offeror. Offerors with no relevant past or present performance history or the offeror's performance record is so limited that no confidence assessment rating can be reasonably assigned shall receive the rating "Unknown Past Performance," meaning the rating is treated neither favorably nor unfavorably. If the prime contractor has no past performance and is relying on their subcontractor's past performance, the prime contractor will be assessed an "Unknown Past Performance." g. In evaluating past performance, the government reserves the right to give greater consideration to information on those contracts deemed most relevant to the effort described in this solicitation. h. If the lowest priced evaluated offer is judged to have a "Sufficient Past Performance" performance rating that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers. The government reserves the right to award a contract to other than the lowest priced offeror if the lowest priced offeror is judged to have a performance confidence assessment of "Limited Past Performance" or when it is in the best interest of the government to do so. i. If the lowest priced offeror is not judged to have a "Sufficient Past Performance" rating, the next lowest priced offeror will be evaluated and the process will continue (in order by price) until an offeror is judged to have a "Sufficient Past Performance" performance rating or until all offerors are evaluated. j. Offerors are cautioned to submit sufficient information and in the format specified in the proposals preparation instructions. Offerors may be asked to clarify certain aspects of their proposal or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors would not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision. k. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest of the government. (End of Addendum) 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS (May 2011) An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (a) Definitions. As used in this provision-- "Economically disadvantaged women-owned small business (EDWOSB) concern" means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program. "Forced or indentured child labor" means all work or service- (1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or (2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties. "Inverted domestic corporation," as used in this section, means a foreign incorporated entity which is treated as an inverted domestic corporation under 6 U.S.C. 395(b), i.e., a corporation that used to be incorporated in the United States, or used to be a partnership in the United States, but now is incorporated in a foreign country, or is a subsidiary whose parent corporation is incorporated in a foreign country, that meets the criteria specified in 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c). An inverted domestic corporation as herein defined does not meet the definition of an inverted domestic corporation as defined by the Internal Revenue Coe at 26 U.S.C. 7874. "Manufactured end product" means any end product in Federal Supply Classes (FSC) 1000-9999, except- (1) FSC 5510, Lumber and Related Basic Wood Materials; (2) Federal Supply Group (FSG) 87, Agricultural Supplies; (3) FSG 88, Live Animals; (4) FSG 89, Food and Related Consumables; (5) FSC 9410, Crude Grades of Plant Materials; (6) FSC 9430, Miscellaneous Crude Animal Products, Inedible; (7) FSC 9440, Miscellaneous Crude Agricultural and Forestry Products; (8) FSC 9610, Ores; (9) FSC 9620, Minerals, Natural and Synthetic; and (10) FSC 9630, Additive Metal Materials. "Place of manufacture" means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture. "Restricted business operations" means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate- (1) Are conducted under contract directly and exclusively with the regional government of southern Sudan; (2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization; (3) Consist of providing goods or services to marginalized populations of Sudan; (4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization; (5) Consist of providing goods or services that are used only to promote health or education; or (6) Have been voluntarily suspended. "Service-disabled veteran-owned small business concern"- (1) Means a small business concern- (i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and (ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran. (2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16). No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. 5352.201-9101 OMBUDSMAN (APR 2010) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency. (e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. (End of clause) USAFA 5352.242-9000, Contractor Access to Air Force Installations (AUGUST 2007) applies to this acquisition. (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and [insert any additional requirements to comply with local security procedures] to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management,] citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. Performance Work Statement: CATHOLIC PASTORAL ASSOCIATE COMMUNITY CENTER CHAPEL PERFORMANCE WORK STATEMENT 12 July 2011 1. DESCRIPTION OF SERVICES. 1.2. Develop, implement, coordinate and evaluate religious education opportunities for families. Coordinate with Chapel staff, Lay Ministers, and the Parish Advisory Council to carry out administrative and other support of all Catholic programs offered and conducted at the United States Air Force (USAF) Academy Community Center Chapel. The Catholic Pastoral Associate works to develop, enhance and expedite a comprehensive Catholic Program, including all necessary internal and external documentation. 2. QUALIFICATIONS. 2.1. Must possess a Bachelors degree in Religious Education or a minimum of five years experience as a catechist in a chapel or church setting. 2.2. Must be Archdiocese of Military Services (AMS) catechist certified. 2.3. Must have experience working with Catholic Children's programs including Sunday R.E., sacramental preparation for Reconciliation and Eucharist, and Children's Liturgy of the Word. 2.4. Must be trained and have experience working in the Rite of Christian Initiation of Adults process. 2.5. Must have experience with recruiting and training catechists and other volunteers. 2.6. Have the ability to tope 40+ WPM and possess a working knowledge of Windows 2007 operating system, Microsoft Word, Excel, PowerPoint, Publisher and Microsoft Outlook (E-Mail) communication. 2.7. Must furnish a resume as part of proposal showing education, experience and personal and professional references. The resume or cover letter must address all areas of the statement of work. 2.8. Knowledge and experience as an Administrative Assistant is required. 2.9. Possess a basic working knowledge of Chapel Tithes and Offering Fund (CTOF) and appropriated funds budget process and procedures. 3. DUTIES AND RESPONSIBILITIES. 3.1. Coordinate with Catholic chaplain to arrange space for Catholic Religious Formation programs, children's church and ceremonies to include but not limited to: First Reconciliation, First Communion, Confirmation, Rite of Christian Initiation for Adults (RCIA), Baptisms, and Special Programs in Religious Education (SPIRE), Vacation Bible School. In carrying out the responsibilities, under the direction of the Catholic chaplain, the associate collaborates with pastoral council officers, volunteer lay leaders and members of the Community Center Chapel. 3.2. In consultation with the Catholic chaplain, develop and submit an annual calendar of events. Make recommendations for the Chapel Tithes, Offering Fund (CTOF), appropriated fund budgets, and annual religious education curriculum order of supplies to the Catholic Chaplain and Wing Chaplain for review and/or approval as determined by local instructions. 3.3. Provide administrative support for the purpose of coordinating and maintaining records for retreats (prep school, lay minister) and spiritual renewal programs. May serve as a Project Officer for but not limited to: Catholic Pilgrimage and mission trips, etc. In this regard, oversee registrations, payments, responds to inquiries, and creates a working folder. Submits and files history reports (after-action reports) for future reference by the Catholic chaplain. 3.4. Submit fund requests, as required, for support of the RE program to the Catholic chaplain. See local HC operating instruction for specifics on submitting CTOF funds requests. 3.5. Submit/ensure/maintain work relating to the stated religious education programs, including family data base, program registrations, class lists, volunteer information, and sacramental record collection and annually recording sacraments for AMS permanent records. 3.6. Keep a record of enrollment and attendance of all Catholic Religious Education activities, services etc. 3.7. Organize classes; recruit volunteer teachers, substitutes, and other necessary staff; trains teachers and substitutes; and provide workshops as required. Training will follow guidelines set forth by Archdiocese for Military Services. 3.8. Plan events and consider ways to honor and recognize the volunteer staff during the year. 3.9. Keep updated on religious and sacramental expectations of the Catholic Church and Archdiocese for Military Services and liaison to the local diocese. 4.0. Meet weekly or as necessary with Catholic chaplain and Chapel staff, to represent Religious Formation on the Catholic Pastoral Advisory Council. A brief written report on Religious Formation activities should be submitted for the minutes. 4.1. Maintains all sacramental records for the Catholic Program; notifies the AMS and ensures relevant parishes receive notice of all sacramental rites conducted at the USAFA Academy Community Center Chapel; records the necessary correspondence and information; maintains records of sacramental preparation completed by those preparing to receive Baptism, Eucharist, Confirmation, and Matrimony; completes and submits the chaplains' quarterly reports to the Archdiocese for Military Services. Also includes maintaining pre-marriage records for submittal to the Archdiocese. 4.1. Maintains continuity folder for the Catholic Advisory Council, to include a roster of council members (subject to Privacy Act), agendas, and minutes for each meeting. Attend these meetings to provide information that will assist members in their deliberations. 4.2. Implement the Catholic Pastoral plan, goals and objectives for Catholic liturgies and other activities in coordination and approved by the Catholic chaplain. 4.3. Coordinates with facility manager, on availability and scheduling the facility for Catholic program events, worship and ceremonies to include but not limited to: reconciliation services, holy day worship services, Lenten observances (such as Station of the Cross, adoration and benediction, and evenings of special devotion), parish picnics/socials, and special events. 4.4. Compiles and prepares each Catholic Sunday bulletin for review by the Catholic chaplain according to the deadline set by the Chaplain Corps support staff. 4.5. In coordination with the Catholic chaplain, provide budget history and recommendations to the Parish Advisory Council and the Account Manager in preparing annual CTOF budget for the Catholic Program. 4.3. Participate in at least one continuing education or professional development event each year, to include travel expenses, may be funded by 10 ABW Chaplain Corps appropriated or Chapel Tithes and Offerings Fund, contingent upon availability of funds and approval by the Wing Chaplain. 4.4. Will personally perform the services described above or coordinate performance by a substitute of equal or higher proficiency who has been approved in advance by the catholic chaplain. Submit the names and qualification of proposed subcontractors to the catholic chaplain for approval at the beginning of the contract award. Subcontractors must meet the requirements for the position as outlined in the Statement of Work (SOW). Contractor is responsible for payment of the substitute 6. GOVERNMENT FURNISHED PROPERTY EQUIPMENT AND SUPPLIES. 6.1. Equipment, supplies, office space and furnishing necessary to complete the duties of this position will be providing by the 10th ABW Community Center Chapel. The contractor will be responsible for returning equipment, supplies and facilities to the location and/or condition received. Any maintenance needs or damage reports must be submitted to the NCOIC, Chapel Operations. 7. GENERAL REQUIREMENTS 7.1. SECURITY REQUIREMENTS. If awarded the contract, the contractor must submit an application for a background check to the NCOIC, Program Support according to the Department of Defense Instruction (DoDI 1402.5), Criminal History Background Checks. 7.2 Performance of Service during Crisis Declared by the National Command Authority: This service is determined to be non-essential for performance during crisis according to DODI 3020.37. However, in the event of a crisis or significant emergency is declared, the CO will contact contractor personnel to notify them of the situation and any possible changes to their contract responsibilities.
