Description
QUICK-FIX STATEMENT OF WORK Project Title: Building 9559 Remodel Project Number: AF Form 332 #25854 Project Site: Building 9559, JBER-Elmendorf 673D POC: George Rockhill 384-1238 or Mike Marberg 384-0216 SCOPE OF WORK: The Contractor shall provide all labor, equipment, materials, supplies, handling and storage, transportation, supervision and quality control to perform all work, including work of an incidental nature, TO REMODEL OFFICE SPACE AND INSTALL A SHOWER ROOM. The work requires the Contractor to provide and install all materials required to complete the work. This project includes, but is not limited to the following work: 1. Remove existing wall, and enlarge office as shown on drawing. 2. Install new steam radiators on both sides of new office walls, with new t-stat and zone valve to control heat in office. 3. Remove existing steel baffling in hush house storage area, floors walls and overhead. 4. Tie into existing plumbing to install new shower area as shown in drawing. Then backfill storage area and pour a 4" slab on grade. 5. Cut 2 doorways in concrete walls where indicated, 1 for entrance to new shower area, and one for new entrance to storage area. And install new doors. 6. Power wash or clean soot from walls and overhead in storage area after the removal of steel and shelving from area. 7. Install new shower area approx. 5X8 where indicated on drawings with a 36" shower enclosure, vent fan, new lights, towel bar and built in linen shelves, small bench and clothes hooks. Install vanity mirror on back of door. Install suspended ceiling in shower area. 8. Fur out walls in storage area and insulate exterior wall. 9. Repaint interior walls of building. (color to be picked by occupant) 10. Install new steam unit heater in storage area with t-stat control for same. 11. Install new grid and ceiling tiles through the whole building. 12. Install new.080 vinyl floor covering and cove base in the building. 13. The mechanical room is NOT to be included in any of this work, with the exception of the heat and possibly the electrical tie-ins. SITE VISIT: It is required that the Contractor visit the work site to verify location and thoroughly familiarize himself with existing conditions prior to submission of bid. APPLICABLE CODES: All work in this contract shall be done in accordance with National Fire Protection Association (NFPA), Uniform Building Code (UBC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), National Electrical Safety Code (ANSI C2), OSHA-Occupational Safety and Health Act, EM 385-1-1, US Army Corps of Engineers Safety and Health Requirements Manual, ANSI B 20.1 1992, 40 CFR EPA, Environmental Protection Agency, and all local safety, fire and security regulations. This job requires that all personnel have appropriate, applicable and current Certificate of Fitness cards from Alaska Department of Labor. All codes listed here shall be the latest version and most recent or up-to-date publications in print. CONTRACTING AUTHORITY: Only the Contracting Officer is authorized to make changes or additions to the contract. The COR and Government Inspector are authorized to act for the Contracting Officer only to the extent that such authority has been delegated to them in writing by the Contracting Officer. COORDINATION OF WORK: Once the contractors' proposals have been received and reviewed, Contracting will notify the contractor awarded the contract. NOTICE TO ALL INTERESTED PARTIES: This requirement will be awarded under the JBER Quick Fix program and will be awarded to one of four prequalified Blanket Purchase Agreement (BPA) holding contractors and is being posted here solely to satisfy the requirement that all requirements with an estimated value of $25,000.00 or greater be advertised to the public.
