- QUICK-FIX STATEMENT OF WORK Project Title: Bldg 690 Office Carpet Project Number: 27976 Project Site: Bldg 690, JBER Richardson, Alaska 673D CES POC: Tim Formo 384-7854/Mike Marberg 384-0216 SCOPE OF WORK: The Contractor shall provide all labor, equipment, materials, supplies, handling and storage, transportation, supervision and quality control to perform all work, including work of an incidental nature, RENOVATE OFFICES, CONFERENCE AREA AND RACKEBALL VIEWING AREAS. The work requires the Contractor to provide and install all materials required to complete the work. This project includes, but is not limited to the following work: OFFICE AREA 1 1. Remove all cabinets, stub walls, plumbing fixtures and associated piping in areas depicted on diagram. 2. Cap plumbing behind walls and under floor, patch and finish to match as needed. 3. Cut into existing wall and match finish to form open archway a minimum of 6’ wide where designated on diagram. 4. Lower outlets to match other office’s height and patch/finish to match. 5. Install 3 LAN drops. 6. Patch and repaint walls in converted Child Care area. 7. Replace all existing floor coverings with carpet (minimum 28oz. level loop commercial grade). 8. Install new 4” cove base in all areas. 9. All colors selected by tenant user. OFFICE AREA 2 1. Demo existing floor sink, shower curtain and faucet. 2. Cap plumbing behind walls and under floor, patch and finish to match as needed. 3. Install 4’ x 4’ sliding window as shown on diagram. 4. Remove air actuator and cover with flush mount grill. 5. Demo floor and replace with.080 heat seam vinyl. 6. Install 4” cove base. 7. Patch and paint walls. 8. Install suspended ceiling with egg grate tile. RACKETBALL VIEW AREA 1. Remove and replace carpet and cove base in upper and lower racketball viewing areas. 2. Carpet to be minimum 28oz. level loop commercial grade carpet squares. 3. Cove base to be 4” vinyl. SITE VISIT: It is required that the Contractor visit the work site to verify location and thoroughly familiarize himself with existing conditions prior to submission of bid. APPLICABLE CODES: All work in this contract shall be done in accordance with National Fire Protection Association (NFPA), Uniform Building Code (UBC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), National Electrical Safety Code (ANSI C2), OSHA-Occupational Safety and Health Act, EM 385-1-1, US Army Corps of Engineers Safety and Health Requirements Manual, ANSI B 20.1 1992, 40 CFR EPA, Environmental Protection Agency, and all local safety, fire and security regulations. This job requires that all personnel have appropriate, applicable and current Certificate of Fitness cards from Alaska Department of Labor. All codes listed here shall be the latest version and most recent or up-to-date publications in print. CONTRACTING AUTHORITY: Only the Contracting Officer is authorized to make changes or additions to the contract. The COR and Government Inspector are authorized to act for the Contracting Officer only to the extent that such authority has been delegated to them in writing by the Contracting Officer. COORDINATION OF WORK: Once the contractors’ proposals have been received and reviewed, Contracting will notify the contractor awarded the contract. NOTICE TO ALL INTERESTED PARTIES: This requirement will be awarded under the JBER Quick Fix program and will be awarded to one of four prequalified Blanket Purchase Agreement (BPA) holding contractors and is being posted here solely to satisfy the requirement that all requirements estimated to have a value of $25,00.00 or greater be advertised to the public.
