Description
Winning bidder will be required to submit these safety and man hour documents before proceeding with award. The U.S. Army Corps of Engineers, Buffalo District is pleased to annouce an opportunity for the purchase, delivery, and full integration of a prefabricated building to house hazardous materials for our Cleveland Area Office site. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The applicable North American Industry Classification System (NAICS) code is 332311. Small Business Size Standard for this acquisition is 500 employees Set-Aside: This procurement is set-aside for 100% small business. Proposal Due Date: Proposals are due by 15:00 EST 16 September 2011. Proposals shall be delivered to Brooke Hundley, Contracting Division, Department of the Army, U.S. Army Corps of Engineers, Buffalo District, 1776 Niagara Street, Buffalo, NY 14207-3199. Or emailed to: brooke.m.hundley@usace.army.mil The scope of work is as follows: Acquisition of Prefabricated Hazardous Material Storage Building Cleveland, Ohio •a. WORK TO BE PERFORMED •a. General Scope of Work. Supplying all necessary materials, equipment, labor, ancillary items and supervision to produce a prefabricated Hazardous Material Storage Building. Designing, constructing, and delivering a prefabricated hazardous material storage building to the Cleveland Area Office, which will be placed on the reservation at the COR discretion. •b. Purpose of Work. To provide a hazardous material storage building for the Cleveland Area Office which meets OSHA, EPA, NFPA UL Design, and ASTM Standards for the storage of hazardous material generated at the reservation. •c. Location of Work: Cleveland Area Office; 1035 E. 9 th Street, Cleveland, Ohio 44114. •d. Description of Work Required. • i. All work bid under this item will be lump sum. • ii. Contractor to prepare and submit a mobilization and placement plan for the installation of the building. Contractor will be responsible for unloading of the building at the facility and placing it in final location as designated by the COR. The building will be placed on either concrete or asphalt ground at the Cleveland reservation. The location of the building will be approximately 250 feet from the source depending on the run of lines. The reservation currently has single phase, 30amp/220V power. Contractor will be responsible for verifying power source before hookup. • iii. The work entails acquiring and placing a prefabricated hazardous material storage building on the Cleveland Reservation to the required specifications: •1. Hazardous Material Storage Building •a. The interior area to be approximately 96 sq. ft. •b. The interior height will be approximately 90 inches. •c. The interior area should be able to accommodate a minimum of 24 Drums. •d. Structure must conform to EPA and OSHA standards for hazardous material storage. •e. Wall structure shall have a two hour fire rating which meets UL 263 & ASTM E - 119 and conforms with NFPA 30. •f. Roof structure shall have a two hour fire rated Class A flame spread rate and constructed of two hour fire rated, weather proof, noncombustible construction same as the building walls. •g. Flooring system shall be a leak proof spill containment system with sump and continuous steel floor grating throughout building. Materials shall be chemical - resistant acrylic alkyd enamel coated or similar. Steel flooring shall conform to NAAMM MBG 531. •h. Building base shall be chemical resistant, with forklift pockets and hold-down braces. The base shall be capable of withstanding 1000 psf minimum. •i. Storage building shall have static grounding. •j. Provide positive ventilation; dampers shall be 1 ½ hour fired rated with a fusible link, to UL specifications. •k. Building shall have permanent D.O.T. metal flip placard with aluminum holder and stainless steel clips affixed to the structure and one pressure sensitive NFPA 704 Hazard Rating Sign. •l. Contractor will connect the building to existing electrical grid on installation. •m. Ensure all hookups for building are present and that the building is functioning as designed. •n. The interior of each active door must be equipped with UL-Listed panic bar hardware to ensure easy egress in the event of an emergency. •o. The building must be equipped with a UL-Listed emergency local alarm system including a panic alarm button and corresponding exterior audible alarm. •e. Timeline: All work must be completed within 100 days of award. •f. Manufacturer's and Installer's Qualifications. The Contractor shall provide the following information regarding the qualifications of the manufacturer and installer: • i. Statement certifying the manufacturer is licensed, bonded, or approved to produce the building. • ii. Statement certifying that the Contractor is licensed, bonded, or approved to install the building. • iii. Statement certifying that the Contractor has been actively installing prefabricated hazardous material storage buildings or done similar work (that may include actual construction of structures) for a minimum of 5 years. If requested, the names and addresses of 5 previous building projects of the type specified. Include the size of each project, the manufacturer's name, and the name and telephone number of a contact person at the project location. • iv. Statements certifying that the supervisor, job foreman or crew chief, and the workers installing the building are qualified workers and have had a minimum of 3 years experience installing prefabricated hazardous material storage buildings or done similar work (that may include actual construction of structures). •g. Performance Required. The labor should be covered for a one year warranty by an agency that has the resources to provide support out of the immediate regional area. •h. Liquidated Damages. If the Contractor fails to complete the work within the time specified in the contract, the contractor shall pay liquidated damages to the Government in the amount of $100.00 for each calendar day of delay until the work is completed or accepted. •i. Safety. Safety is a key operating principle for the USACE, and, therefore, the Contractor shall adhere to the application of all applicable local, state, and federal requirements. The Contractor will comply with the USACE Safety & Health Requirements Manual (EM385-1-1) which is available on-line at: http://www.usace.army.mil/CESO/Pages/EM385-1-1.aspx This is a Service Contract requiring no field work. An Accident Prevention Plan (APP) is not required at this time. Prior to any work associated with this contract, the Contractor is required to develop the submittals noted below and subsequently submit them as required. Required Safety Submittals: Competent Person/Site Safety Officer (SSHO) Qualifications Level 2:A minimum of 3 years safety work on similar project; 30-hour OSHA construction safety class; competent person training as needed. Present qualifications in resume format with certification(s) included •· (AHA) Activity Hazard Analysis for each phase of the project (Attachment 1) •· Contractor Monthly Man-Hour Reports by the 5 th day of each month (reference Attachment 1) •· Eng Form 3394 Accident Investigation Report as incidents occur (reference Attachment 1) also available at www.mvs.usace.army.mil/safety/ eng3394.pdf All Contractor accidents, involving injuries, illnesses or property damage, occurring during the execution of this contract shall be reported to USACE within 4 hours of the contractor becoming aware of its occurrence. Reporting injuries, illness or property damage to the Contracting Officer Representative (COR) may be sufficient. Based upon the severity of the injury, illness or property damage, more detailed reporting may be required. More detailed notification will include contractor name; contract title; type of contract; name of activity, installation or location where accident occurred; date and time of accident; names of personnel injured; extent of property damage, if any; extent of injury, if known, and brief description of accident (to include type of construction equipment used, PPE used, etc.). Warranties. • i. The warranty must cover all work that is done. The length of the warranty shall extend one year past the date of final acceptance by the Government for all labor. The warranty on the structure/building will be a minimum of 5 years. The warranties will be delivered to the Government no later than the final pay estimate and will be transferred to the USACE Buffalo District. • ii. The Contractor shall issue a warranty for a minimum of 2 years for workmanship and quality over all work performed under this contract. •k. Required submittals: (include requirements, procedures and sample plans/checklists) • i. Quality Control Plan • ii. Schedule • iii. Drawings • iv. Product Data •l. Completion of Work. Completion of work shall include submittal and approval of design drawings and copies of the warranty provided to the government. Work shall be completed within 100 days from award of contract. •m. Final Inspection. The work shall be inspected for conformance to the specifications. Any materials incorporated in the work that was delivered to the site damaged by re-handling, hoisting or otherwise delivery, handling, installation shall be removed from the site and replaced at no additional cost to the Government. In addition, damage to the buildings, grass areas, and fencing shall be repaired to the satisfaction of the Government or replaced in kind. A follow-up inspection will be performed at the following intervals after completion of the work and at 6 months. QUALITY OF MATERIALS All materials, supplies, or articles required for work shall be standard products of reputable manufacturers and suitable for the intended work. This contract will be subject to the clause : 52.225-9 BUY AMERICAN ACT-CONSTRUCTION MATERIALS (JAN 2005) RISKS AND LIABILITIES The Contractor shall assume all risks in connection with performance and delivery of the work and shall be liable for and hold the Government harmless from any and all damages to persons and properties. The contractor shall provide certificates of insurance, a copy of current Workers Compensation certificate and optionally a Performance Bond (if requested). WORK HOURS All work must be performed within the hours of 0700 to 1530 on Monday through Friday. Work done outside these hours must be agreed upon beforehand. Bidding contractors must state in their bid if they request to work outside the specified times above.
