- Subcontrating Plan Sample Client Notification Letter Sample Bank Notification Letter Business Management Questionnaire Statement of Work Cover Letter The Federal Bureau of Prisons, Transitional Drug Abuse Contracting Office, Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services. The contractors will provide services within a ten mile radius of the U. S. Post Office located at 1233 S Washington Avenue, Saginaw, Michigan 48603. Performance periods under this contract will be for a one year base period to begin on November 1, 2011 with four (4) one-year option renewal periods with services ending (if all options are renewed) on September 30, 2016. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three, and Option Year Four respectively. The Following services are required and may be sub-contracted to another licensed contractor. The requirements for Transitional Drug Abuse Treatment and Mental Health Services for male and female Federal offenders include: Base Year: Estimated number of inmates = 35 Substance Abuse Treatment Services: Intake Assessment and Report (one per inmate) for 25 inmates = 25 assessments Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 25 =1,200 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 25 = 1,800 units Family Counseling Sessions (30 minutes = 1 unit) 16 units (per inmate) x 4 = 64 units Individual Transportation (1202) = 4 Trips per week x 52 weeks = 208 Trips Mental Health Treatment Services: Mental Health Intake and Assessment & Report (one per inmate) for 10 inmates =10 units Medication Monitoring (1 units per inmate) for 10 inmates = 10 monitoring units Mental Health Individual Counseling (30 minutes =1 unit) 48 units per inmate for 10 inmates = 480 units Mental Health Group Counseling (30 minutes =1 unit) 72 units recent Statement of Work, dated April 2011 No Master's Required. The request for quotation will be available on or after September 8, 2011 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the quotation will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about September 28, 2011. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
