Description

This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. However, all proposals received prior to September 16, 2011 will be considered by the Government. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. NAICS code is 541511 and the size standard is $25 million. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 Only emailed requests received directly from the requester are acceptable. The list of contract line item number(s) and items, quantities and units of measure, (including option(s), if applicable): CLIN w/DESCRIPTION/ESTIMATED QTY OVER LIFE OF ORDER/UNIT/UNIT PRICE/ESTIMATED TOTAL AMOUNT 0001 1 EA $___________________ $____________________ Medtronic Paceart will provide Paceart Optium Relational Database Software for the VA National Cardiac Device Surveillance Center to include server database Paceart Optium software on CD and 31 concurrent workstation licenses. Licenses will be used in both VA San Francisco and VA Washington DC locations to operate the Paceart relational database. Paceart Optium desktop client workstation software will provide connectivity to the existing Paceart specialized trans-telephonic ECG devices used that provide the ECG recordings of pacemaker telephonic pacemaker transmissions. Paceart Optium will also provide connectivity for the VA Washington DC Cardio Voice Paceart software to the relational database. Medtronic Paceart will provide the Paceart Optium relation database software and also an excel or electronic copy of the database structure table and field name changes from the existing Paceart Get Connected III used by the VA now. Medtronic Paceart will update the existing Paceart Customizations used by the VA, Paceart account 24 (DC) and 724 (San Francisco) to work with the new Paceart Optium software to include customized VA letters, VA reports, and check box for patients unplugging wireless monitors. Letterheads showing the National ICD Surveillance Center will be changed to the VA National Cardiac Device Surveillance Center. Medtronic Paceart will provide additional customization letters for St Jude wanded devices and St Jude wireless pacemakers. Existing letters for TTM patients will display all Paceart TTM appointments. Detailed customization letter examples will be provided by the VA direct to Medtronic Paceart. Medtronic Paceart will electronically import St Jude Corporation patient transmissions as it does with Boston Scientific latitude or Medtronic CareLink transmissions. Medtronic Paceart will provide elearning module for VA Staff to become knowledgeable in new Paceart Optium client workstation software prior to install of new Paceart Optium software. 0002 1 EA $___________________ $____________________ ITEM DESCRIPTION Medtronic Paceart will provide on-site installation service at VA San Francisco for the server upgrade of the existing Paceart Get Connected III to the new database format for Paceart Optium data structure using SQL commands as required of the backend SQL data tables. Paceart will assist with the install of the backup copy of the database to the Washington DC server. Medtronic Paceart will provide on-site training for the VA staff personnel for 2 days in the National Cardiac Device Surveillance Center, VA San Francisco and for 2 days in the Eastern Pacemaker Surveillance Center, Washington, DC.
