Description
WORK STATEMENT AND REQUIREMENTS 1.The Contractor shall provide regular and systematic examinations, 24 hour emergency call back service, preventive maintenance, maintenance and repair to the (4) Passenger Elevators, (1) Freight Elevator, (1) Handicapped lift and (1) dumbwaiter identified in the Schedule of Elevators. Contractor shall furnish all Labor, equipment and materials necessary to keep the identified Elevators and associated equipment in first class condition and proper working order. 2.The Contractor shall maintain the performance set forth in this specification at all times, ensuring that the operational system shall function to the standards specified, including any changes and/or adjustments required to meet varying conditions of hospital occupancy. All equipment, materials and installation shall conform to the AMERICAN NATIONAL STANDARD SAFETY CODE FOR ELEVATORS, DUMBWAITERS, ESCALATORS, AND MOVING WALKS - ANSI, A17.1-1979 and AMERICAN STANDARDS PRACTICE FOR INSPECTION OF ELEVATORS MANUAL - A17.1-1979, including any updates or revisions of said codes. 3.Service shall include, but shall not be limited to, the following: a.Cleaning, lubricating adjusting, repairing and replacing of all parts as necessary to keep the equipment in first class condition and proper working order. b.Furnishing all lubricants, cleaning materials, tools, and parts required. 4.All Work required shall be performed during the Medical Center's regular administrative hours (8:00 a.m. to 4:30 p.m.); Monday through Friday), unless called back for emergency service outside of normal business hours. Contractor shall provide emergency call back service as set forth in this specification. 5.All travel costs associated with the performance of this contract are the responsibility of the contractor and are provided at no additional cost to the Government as part of this contract. Travel costs are defined as but are not limited to time and vehicle costs for personnel in travel to and from job site for all duties outlined in this specification, normal shipping charges on any parts or materials covered by this contract, costs associated with the normal shipping of parts or equipment out for repair that are covered under this contract, etc. SCHEDULE OF ELEVATORS LOCATION OF ELEVATORS ELEVATOR NUMBERTYPE Building #11&2Passenger, 4000 Lb Cap. Atlas (Hydraulic Automatic) 6.This maintenance and repair service shall not include the performance of any work required as a result of improper use, accident, or negligence for which the contractor is not directly responsible. II. PREVENTIVE MAINTENANCE AND INSPECTIONS (PMI) 1.PMI's shall be performed weekly on all (5) elevators and monthly on the dumbwaiter. Contractor shall provide as a MINIMUM 1.5 hours of PMI per elevator each week and 1.5 hours of PMI on the dumbwaiter each month. This is to be done above and beyond his/her repair time each week. PMI's are to be performed during administrative hours on the same day of each week. The contractor shall submit, at the beginning of the contract, to the Chief Engineer, the day of the week he/she intends on performing the required PMI. If the day falls on a holiday the contractor shall perform the required PMI's on the next business day following the holiday. The contractor shall notify the Chief Engineer if he/she intends on changing the PMI day or if he/she cannot perform the PMI ON schedule. 2.PMI's should ensure a safe and efficient level of operation, and all work relative to cleaning, lubricating and adjustment of equipment that is necessary for the desired level of operation shall be performed by the contractor. Any PMI required by the Equipment Manufacturer, and/or applicable laws, regulations, rules, ordinances, codes, etc, but not specifically identified by the Government, shall be performed by the contractor at no additional cost to the Government. In addition, the contractor shall perform the following preventive maintenance and all necessary cleaning, lubricating, and adjusting pursuant to such maintenance at the frequencies identified. Frequencies shall be modified if deemed necessary by the Chief Engineer. ? A. ELECTRIC ELEVATORS Weekly/Monthly Maintenance: HOISTWAY & CAR TOP"Check lube and lubricators for car and cwt rails. "Observe bolts, joints, and brackets. "Check/lube O.H. sheaves and bearings. "Observe all HOISTWAY switches and vanes. "Observe cwt. Shoes, rods and cable length. "Observe Conduit, fittings, and fastenings. "Check tension and hitch of hoist cables. "Check guide shoes, gibs, rollers, and tires. "Check door operator and retiring cam. CAR"Check car light, stop sw.and alarm bells. "Check operating devices and signal lamps. "Check operation of car doors and gates. "Check door or gate contact, safety edge and photo electric beams. "Observe stop and leveling at all floors. CORRIDOR"Test operation of all doors and gates. "Test operation of interlocks and contacts. "Check hall buttons and indicator lamps. Quarterly Maintenance HOISTWAY & CAR TOP"Clean and tighten all belts, brackets and fastenings in HOISTWAY. "Clean and lube all HOISTWAY switches. "Clean and check contacts and actuators. "Clean and lubricate HOISTWAY door hangers. "Clean and adjust door operator contacts, motor brushes, belt, and chain, cords, and clutch and safety edge. CAR"Check and tighten all bolts on sling. "Check fastenings and bolts for cams, safety, cords, and compensating chain. "Check Conduit and electric wiring. PIT"Test buffers and pit stop switch. "Check tail sheave, bolts, hangers, and bearings. Semi-Annual/Annual Maintenance"Brush and vacuum clean HOISTWAY. "Conduct annual safety test. "Clean bottom gibs of car and hall doors. "Test all door hangers for up thrust. "Check lubrication of ball and roller bearings. B. DUMBWAITER and CART LIFT Monthly Maintenance Door & Gate Operation"Clean, lubricate, and adjust brake and operation braking system. "Check linkages, gears, wiring, and motor. "Check keys, set screws, contacts, chains and cams. Door Closer "Clean, adjust and lubricate pivot points, sill trips and checking devices. CAR"Clean, adjust and lubricate car doors, gate tracks, pivot, hangers, car grill, stile channels, side and top exits. "Check adjustment of car shoes, roller guides, and adjust if necessary. "Inspect safety parts, pivots, set screws and switches. Leveling"Clean, adjust and lubricate leveling switches and leveling switches, HOISTWAY vanes, magnets and inductors. HOISTWAY Doors"Clean, lubricate and adjust tracks, hangers and up thrust. Machines"Observe brushes for arcing and gear care bearings for wear and end play. "Inspect worm and gear backlash, thrust and play and any bearing wear in machine. Ropes "Inspect all fastenings and ropes for wear and lubrication. "Advise Contracting Officer when hoist ropes should be replaced. Motor and Generators"Clean all commutators. "Renew or reseat brushes as necessary. "Check elevator shaft for settling and cracks in building structure. "Fill and adjust checks and door eccentrics. "Check bottom gibs, structs, sills, headers, bumpers, and fastenings. "Adjust door contacts as required. Hall "Inspect hall button contacts, springs, and wiring. "Clean and lubricate. Motor and Generators"Clean armatures, and motors with blower or vacuum. "Inspect armature and rotor clearances. "Check motor and MG set connections, change oil in bearings and lubricate in accordance with manufacturer's instructions. Miscellaneous"Clean car grills on stile channels. "Group supervisory control systems where installed shall be checked. "The system's dispatching, scheduling and emergency service features shall be tested and adjusted in accordance with manufacturer's specifications as required. "Keep the exterior of the machinery clean. "If previously painted keep it painted and presentable at all times. C. HYDRAULIC ELEVATORS Monthly Maintenance Check Points"Observe operation of elevator throughout its full range and at all floors it serves to test controls, safety devices, leveling, re-leveling and other devices. "If creeping exceeds 2", determine cause and correct it. "Check opening and closing of doors, gates, the indicators in cab and at each floor. "Inspect interior of cab. "Test normal and emergency lights, fan emergency call system or alarm, miscellaneous hardware, control panel, etc. "Inspect HOISTWAY, pit and machine room. "Clean and lubricate equipment and clean pit and machine room floors. Test mechanism. "Observe operation of motor and pump, oil lines, tank, controls, plunger, packing, etc. "Test manual and emergency control. "Clean and lubricate floor selector drive tapes and chains. "Clean and lubricate automatic show-down and stopping switches on top of cabs on elevators. "Clean car position indicators, signal boards, and adjust if necessary. "Clean and lubricate guides rails, if required. C. HYDRAULIC ELEVATORS Cont. Quarterly Maintenance Clean, Lubricate and Adjust"Clean, lubricate and adjust overload mechanism. "Clean all motors. "Check dispatching systems. "Repair and adjust electronic door operation system, clean as necessary. "Clean mechanism, HOISTWAY top and bottom of cabs, etc. "Inspect and clean car door catch contacts and adjust if necessary. Miscellaneous"This service is to be performed as required to maintain a safe and efficient operation. "Clean and adjust floor selector contacts. "HOISTWAY doors, car gates, and associated equipment are maintained continuously. "Inspect and repair or renew traveling cables. 3.The contractor is responsible for replacement of all light bulbs, including lights in elevator shafts on top and bottom of cars, HOISTWAY crawl spaces, elevator machine rooms, etc. This shall include all lights inside the elevator cabs. 4.The contractor shall during regular PMI visits retrieve any objects such as key, wallets, glasses, etc. dropped in elevator pit by occupants of the building. Such objects shall be turned over to VA Police personnel. 5.The contractor must maintain the efficiency, safety and speeds specified in the contract or as designated by the manufacturer of the elevators at all times; including acceleration, retardation, contact speed in feet per minute, with or without full load, floor to floor time and door opening and closing time. III. INSPECTION AND TESTING 1.The Annual and Semi-Annual inspection testing required by code are not a part of this contract. These inspections shall be performed by a separate inspection contractor in accordance with VA policy. The maintenance contractor shall be notified of the scheduled inspection and is required to have the regular maintenance mechanic present during said inspections. 2.A copy of the Semi-Annual and Annual inspection shall be furnished to the contractor for correction of identified deficiencies as per section IV, REPAIRS, of this specification. In addition, the maintenance contractor shall increase frequency of PMI's and/or add to the PMI schedule hose items identified by inspection contractor. The maintenance contractor shall submit to the Chief Engineer, within two (2) weeks after receiving inspection report, certification that all deficiencies have been corrected. The contractor may submit for a time extension as covered under Section IV, REPAIRS, of this specification. IV. REPAIRS AND EMERGENCY CALL BACK 1.Vendor shall have technician on site within two (2) hours of request by telephone or otherwise from contacting officer, Chief Engineer and/or designees for emergency services should a shut down or emergency trouble develop between regular examination. Overtime emergency call back service shall be limited to minor adjustment and repairs required to protect and immediate safety of the equipment and persons in and about the elevator. 2.The contractor shall be responsible to make all repairs and adjustments necessary. Repairs are defined as unscheduled work required to (1) prevent a breakdown or malfunction of an item of equipment, (2) a system or a subsystem back in service after a breakdown or malfunction. 3.Repairs shall be completed within 24 hours after the need for repair has been identified by the contractor or by the Government. All repairs shall be in accordance with the equipment manufacturer's requirements and/or applicable laws, regulations, rules, ordinances, codes, etc. a)A time extension may be granted on a case by case basis upon receipt of: 1.A request for time extension by the contractor (within) the 24 hour requirement) with an explanation for the delay. 2.Contractor's request includes an estimated time of completion of repair. 3.Evidence clearly shows that the contractor has made effort to comply with the time requirements. V. SUPPLIES, NATERIALS, AND REPLACEMENT PARTS 1.Except as noted elsewhere in this specification contractor shall furnish all labor, materials, supplies, lubricants, cleaning materials and parts necessary to perform cleaning, maintenance, inspection, and repairs to elevators, equipment, appurtenances, and accessories. 2.All parts and materials shall be of the original manufacturer's design and specification, or equal thereto. All lubricants shall be as recommended by the manufacturer. 3.The contractor shall have and maintain on hand locally a supply of spare parts sufficient for normal maintenance and expedient emergency repairs of the elevators. 4.Reasonable space in the elevator machine room (8th floor Penthouse) will be assigned to the contractor for the storage of his bulk supplies, replacement parts and the equipment which he will use in the performance of work under the contact. The contractor shall maintain this space in a neat and orderly condition. The area will be secured at all times with station control key and lock. The Government will not be responsible in anyway for damage to the contractor's store supplies, materials, replacement parts or equipment. 5.The following items are specifically excluded from this contract: a)The refinishing, repair to and/or replacement of elevator cabs, floor coverings, HOISTWAY enclosures, HOISTWAY door panels, entrance frames and sills. b)Replacement or repair parts, where such parts have a purchase value to the Government (excluding installation labor) of $200.00 or more and the labor to install such parts. Contractor shall refer to Section VIII, paragraph 1, of this specification. THIS EXCLUSION IS FOR REPLACEMENT AND/OR REPAIR PARTS OLY. The exclusion does not relieve the contractor of providing necessary lubricants, oils, equipment, tools, etc., at no cost to the Government that have a purchase value greater than $200.00 VI. RECORDS AND REPORTS The contractor shall instruct his personnel that when they perform work under this contract, they shall comply with the following procedures: 1.Upon arrival at the station, the contractor shall check in with the Chief Engineer or his designee. 2.Contractor shall sign-in/obtain an ID Badge at the operator/switchboard during administrative hours (8am to 4:30pm, M-F). If arriving after hours, contractor shall sign-in at the Boiler Plant. 3.CAT9 - Upon completion of the work, the contractor shall sign out in the previously method noted above. In addition the contractor shall furnish the Chief Engineer or his designee a copy of a work order, service sheet or other such written notification of services performed. Such reports shall include the following information: a)Name and address of contractor b)Name of contractor employee(s) performing the work c)Date(s) work performed and hours spent. d)Brief description of work performed, including PMI's and identification of equipment worked on. e)Signature of contractor's employee(s) and signature of Chief Engineer or designee. 4.A copy of the before mentioned report shall be left with the Chief Engineer or his designee. If report is not submitted it will indicate that no inspection or maintenance was performed and payment may be withheld and/or delayed. The contractor shall maintain a separate maintenance record on each elevator and dumbwaiter. The maintenance record shall be kept in the elevator equipment room and shall be readily available for inspection by the Chief Engineer or his designee. The maintenance record is the property of the government and shall be turned over to the Government at the end of the contract. The maintenance record shall be a chronological file that includes the following information: a)Date. b)Nature of work (i.e. PMI or call back). c)Description of work performed (contractor should be brief and concise.). d)Any indication of problems experienced. e)Initial or signature of mechanics performing work. VIII. SAFETY 1.Patient, employee, visitors, and contractor personnel safety shall be maintained at all times. The contractor is responsible for the occupation safety and health of his/her employees. The contractor shall comply with all applicable OSHA safety and health standards. The contractor shall replace all safety guards, equipment, devices, etc. removed to service or repair the elevator immediately after completion of work or when leaving the job unattended. 2.The contractor shall submit to the Safety Office for approval, the Contractor's Safety plan and MSDS sheets for all materials and products used requiring MSDS sheets. The contractor shall maintain a copy of the final VA approved safety plan and all required MSDS sheets in the 7th floor penthouse and is required to assure compliance with all laws and requirements regarding the "Right to Know" law. 3.The contractor shall obtain all required Hazardous Activity or Work Permits from the Safety Office whenever soldering, welding, using a cutting torch, or other open flame, spark, or heat producing equipment, working in confined spaces. The contractor is required to follow ALL requirements outlined for the issuance of the Permit. 4.The contractor shall be notified of any non-compliance with the pervious provisions. The contractor shall, after receipt of such notice, immediately correct the condition and notify the contracting officer in writing that the condition has been corrected. It, in the opinion of the Facility Safety Officer or designee the condition is life threatening he/she shall instruct the contractor to stop work until the condition has been corrected. The contractor bears all costs of stop work time and correcting safety hazards. IX. GENERAL INFORMATION 1.The contractor shall immediately notify the contracting office and contracting officer's representative (in writing) of the existence of the development of any defects in, or repairs required to the elevators which the contractor considers he is not responsible for under the terms of the contract, and shall furnish him a written estimate of the cost to make necessary repairs. The contracting officer reserves the right to make final determination of the responsibility. The contacting officer also reserves the right to solicit the services of other contractors to make said repairs. 2.The Government reserves the right to perform emergency repairs to any elevator which is disabled if, in the Government's opinion, it jeopardizes patient or employee safety. 3.The contractor shall ensure that the elevator pits(s) and elevator machine room(s) are kept clean and free from dirt and debris. Contractor shall clean and vacuum all HOISTWAY(s), pit(s) and machine rooms once a month to remove dust and debris accumulation. 4.The contractor shall require his employees to comply with all VA rules and regulations pertaining to the conduct of his employees while on station. Particular attention is drawn to the fact that the VAMC Manchester is a NO SMOKING Facility. Smoking is not permitted in any building (including Penthouses and elevator pits). 5.Contractor shall have use of station telephones to make business calls related to Medical Center's equipment. Misuse of this privilege will necessitate the need to require the contractor to use public telephones. ? ELEVATOR MAINTENANCE SPECIFICATIONS SPECIAL CONDITIONS I. QUALIFICATION OF BIDDERS 1.Upon request of the Government, Bidder shall be able to show evidence of his reliability, ability and experience by furnishing; (1) A list of personnel who will perform under the contract showing the length and type of experience of such personnel and (2) the names and addresses of other concerns and/or Government Agencies for which prior comparable services were rendered by the bidder. Generally, the bidder shall have had approximately 5 years successful experience in fully inspecting elevators. 2.Ability to meet the foregoing experience requirements and the adequacy of the information submitted will be considered by the Contracting Officer in determining the responsibility of the bidder. II. INSPECTION OF PREMISES 1.Bidders should inspect the premises prior to submitting bids in order to be fully aware of the scope of service required. Failure to do so will not relieve the successful bidder from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. Prospective bidders should call the Contracting Officer and COTR to arrange an appointment to inspect the premises. III. QUALIFICATIONS OF MECHANICS 1.All work under this contract shall be performed by an Elevator Mechanic. Elevator Mechanic Helpers or Probationary Helpers will be allowed to assist in maintaining specified equipment only in the presence of an Elevator mechanic. Helpers will only be allowed to work themselves if performing lubrication or cleaning service. CONTRACTORS SAFETY REQUIREMENTS The contractor shall adhere to all OSHA, EPA, NFPA Life Safety Codes, and all other regulator requirements. The contractor is responsible to identify, provide and maintain all personal protective equipment required to perform the duties outlined in the contract. In addition, the contractor is responsible for identifying and providing all applicable safety programs (i.e. lockout/tag out, confined space entry, universal precautions, etc.) required to perform the work. Training on required safety programs and the proper use of PPE's shall be provided, and documentation maintained by the contractor. The contractor shall submit MSDS'S for approval of the Chief Engineer prior to utilizing any chemical on Medical Center Property. The government retains the right to require a less hazardous chemical of comparable nature and cost, at no additional cost to the Government. Contractor is responsible for maintaining MSDS'S on site for all products used and stored on site. The contractor shall provide within ten (10) working days after receipt of this contract a detailed listing of the following information: a)Safety program/procedures are required to be followed by the contractor's personnel in the performance of their duties and when such procedures are required. b)Listing of personal protective equipment(s) required to be utilized by the contractor's personnel in the performance of their duties. Also when equipment will be required. c)Material Safety Data Sheet (MSDS'S) for any chemical(s) utilized by the contractor in the performance of this contract. "Hot Work Permit" issued by the Medical Center's safety staff, are required to all hot work (i.e. welding, soldering, brazing, open flames or any other work with may be and ignition source.) Contractor is responsible for the supervision of all their employees while on government property. It is the contractor's responsibility to assure compliance with the scope of work and requirements referenced in this contract. Provisions provided in the scope of work are not intended to relieve the contractor of this responsibility
