- Scope of Work (SOW) - 2011 March Evaluation: A.Provide an on-site survey evaluation of approximately thirty-seven (37) elevators/lifts to determine operating performance based on engineering practices common to the vertical transportation industry and current evaluation report. B.The evaluation shall be conducted in accordance with the latest editions of the American Society of Mechanical Engineers, ASME A17.1, ASME A17.2, ASME A17.3 and Connecticut Laws/Rules Regulating Elevators. C.The evaluation shall include monitoring Category 1 test requirements: Newington Campus; Buildings 1, 2E, 3 a total of 8 electric elevators, West Haven Campus; Buildings 2, 5, 24, 35A a total of 6 hydro elevators. D.The evaluation shall include a review of the existing equipment for safety code compliance, firefighters' service, ADAAG requirements, recommended upgrades, enhancements and/or modernization. E.The evaluation shall include a review of the pedestrian/employee usage of the equipment to determine the way and how the elevators are used and/or operated. F.The evaluation shall include review of the present preventive maintenance program, to determine coverage, terms; price and conditions in the form of frequency, technical approach and repair technique. GThe evaluation shall include review of the facility's disaster plan to determine its effectiveness when elevators are under emergency conditions. When waiting for elevator personnel is impractical. D.The evaluation shall include a review of the existing equipment for safety code compliance, firefighters' service, ADAAG requirements, recommended upgrades, enhancements and/or modernization. E.The evaluation shall include a review of the pedestrian/employee usage of the equipment to determine the way and how the elevators are used and/or operated. F.The evaluation shall include review of the present preventive maintenance program, to determine coverage, terms; price and conditions in the form of frequency, technical approach and repair technique. GThe evaluation shall include review of the facility's disaster plan to determine its effectiveness when elevators are under emergency conditions. When waiting for elevator personnel is impractical. H.Each evaluation shall include a meeting with Facilities Services to discuss immediate needs and needs based on long term life expectancy. I.Each survey evaluation shall include VA Forms; 10-6166/10-6167 along with a comprehensive report. The report shall include a current listing of the operating performance for each elevator/lift and a narrative, which summarizes the inspection/tests conducted, identifies conditions found to exist, provide recommendations as to what actions should be implemented to correct deficiencies and/or conditions identified. Reports shall be submitted electronically within two (2) weeks after completion of the survey evaluation. II. Contractor shall complete all inspections and evaluations during the visit which will take approximately 5 days. If inspections are delayed the inspector will work accordingly with elevator maintenance contractor to ensure all work is completed. In some cases the work will require extra hours. III. Inspector must be available to visit VA campus 2 weeks upon notification of contract award. Evaluations of elevators may require an emergency visit, QEI inspector must be made available at VA's request to discuss current conditions or emergency site visit within 1 weeks notice.
