Attachment 2, BAA Certificate Attachment 1, Bar Code Sample This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written (formal) solicitation will not be issued. This solicitation is issued as a Request for Quotation, (RFQ), solicitation number FA7000-11-T-0114, in accordance with FAR Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-53 effective 04 Aug 2011. The North American Industry Classification System Code (NAICS) is 316213 and the business size standard is 500. This is a 100% total small business set-aside. All responsible sources may submit a quote, which if received timely, will be considered by the 10th Contracting Division, USAF Academy (USAFA). Please read and comply with all requirements for submitting a quote. Failure to comply with all instructions contained within this combined synopsis/solicitation could result in the quote being ineligible for award. It is the Offeror's responsibility to ensure their quote meets all the requirements identified herein. The USAF Academy has a Brand Name or Equal requirement to purchase Capps Shoe Style 90253 Colonel (Men's) and Capps Shoe Style 90105 Rita (Women's) or equal products with the salient characteristics provided below. The LRS/LGRC determined the only suitable features to provide functionality they require. Initially, quotes will be evaluated on a pass or fail basis (Brand Name or Equal, Provision at FAR 52.211-6) to determine whether the proposed dress shoes meets the salient, physical, functional, or performance characteristics of the specifications provided at the lowest evaluated price. b. Quotes will then be ranked according to price. An offeror's proposed prices will be determined by multiplying the quantities identified in the Pricing Schedule by the unit price for each item to confirm the extended amount for each line item. The offeror who submits the lowest evaluated price and meets or exceeds the salient physical, functional or performance characteristics will receive the award. The award will be made to that offeror without further consideration of any other offerors. c. The Offeror of an "or equal item" must submit data that adequately demonstrates that its product meets the salient characteristics of the solicitation. A general statement of compliance or restatement of the salient characteristics is insufficient. The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract.
