Description
This is a COMBINED SYNOPSIS/SOLICITATION. The Regional Contracting Department NAVSUP Fleet Logistics Center Pearl Harbor, Hawaii intends to award a 100% small business set-aside firm fixed price contract under the procedures and provisions of FAR Part 12 Commercial Item and FAR Part 13 Simplified Acquisition Procedures. The proposed contract action is for commercial supplies. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/). The RFQ number is N00604-11-T-3228. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-53 and DFARS Change Notice 20110725. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acqnet.gov/far and http://www.acq.osd.mil/dpap/dfars/index.htm. The NAICS code is 339113 and the Small Business Standard is 500 Employees. All prospective vendors providing quotes are responsible for monitoring this site for the release of amendments (if any) or other information pertaining to this solicitation. The NAVSUP Fleet Logistics Center Regional Contracting Department requests responses from qualified sources capable of providing: CLIN 0001, Fifty (50) Exam Tables, custom, State-of-the-Art used for the examination of secured Forensic skeletal and dental evidence. Table specifications are listed below. List of specifications: Exam Tables with lockable wheels. Wheels should be at a minimum of 2 inches and no more than 4 inches. Table will be 34 inches high with adjustable heights of 32 inches and 30 inches. Table Top Surface Area will be 40 inches by 90 inches with an additional 2 inch wide rim round the perimeter (total dimesions will be 44 inches by 94 inches). The rim securely prevents any skeletal or dental remains from falling from the table. The rim will 1.5 inches high and 2 inches wide all around and non-beveled. Table will consist of mild steel that will hold a powder coat and be able to handle capacity of 500 pounds. Table should not look like an autopsy table. Underslung from the table there needs to be an open rack or basket on either end to hold things like boxes of gloves, calipers, some forms. Drawers under the table will not suffice. This can be under-slung from the table top. We need to be able to sit at the table and get our legs under it. At the foot of the table we would need a metal strip so as to be able to magnetically attach a placard that holds the case information for the table. The table top will not handle any liquids or body fluids. All quotes shall include price(s), FOB point, a point of contact, name and phone number, GSA contract number if applicable, business size, Tax ID#, cage code, and payment terms.
