- Attachment 2 Line Item Spreadsheet Attachment 1 Statement of Work (SOW A. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The requirement is for DoD Education Activity DDESS School, West Point Middle School, located in West Point, New York. This solicitation uses Simplified Acquisition Procedures (SAP) authorized in FAR Subpart 13.5 and is expected to result in the award of one firm-fixed unit priced contract using commercial contracts standard format 1449. This announcement constitutes the only solicitation; quotations are being requested, and a separate written solicitation will not be issued. B. HA603211RCV7140 is a request for quotation (RFQ): its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-53. This procurement is 100% set aside for Small Business. NAICS CODE: 334310. Size Standard: 750 Employees. C. PROCUREMENT is for the removal of existing sound system in the auditorium of the West Point Middle School located in West Point, New York, and to provide and install a new audio visual system in its place to include all equipment, materials, shipping and travel costs. D. DESCRIPTION OF REQUIREMENT. (1) Attachment 1 is the Statement of Work (SOW). (2) Attachment 2 provides a list of required items and their quantities. The attachment is a spreadsheet. Complete the attachment with your prices to include discounts (Federal, Educational, other) if available. If an item is not separately priced, please indicate so with "NSP". E. SUBMISSION OF QUOTE. (1). Responses to this RFQ are due no later than 10:00 AM EST on 28 September 2011. (2) Quotes shall be submitted by EMAIL to Nina.Lin at nina.lin@hq.dodea.edu, with the subject line "West Point AV". No other method will be accepted. They shall include the following: (i) Price Quote (Completed Attachment 2) (ii) Cage Code and Federal Tax Identification Number (TIN) (iii) Completed FAR Clause 52.212-3,"Offeror Representations and Certifications-Commercial Items" or a statement indicating that they have been completed online at https://orca.bpn.gov. (3) RFQ QUESTIONS concerning this requirement must be submitted in writing by e-mail with the subject line "West Point AV Questions" to Nina Lin at nina.lin@hq.dodea.edu no later than 10:00 AM Eastern Standard Time 26 September 2011. Questions will not be responded to after that date. F. AWARD. The Government will award a firm-fixed priced order to the lowest priced technically acceptable vendor. See Section H(2) below. Quotes for less than items solicited will be rejected as non-conforming and unacceptable. G. PAYMENT - ELECTRONIC INVOICE SUBMISSION. DoDEA has implemented Wide Area Work Flow Receipt and Acceptance (WAWF-RA) as its method of electronic submission of invoices and government inspection and acceptance. WAWF instructions will be provided at time of award. H. CLAUSES. The following clauses/provisions in their latest editions apply to this solicitation: (1) FAR 52.212-1, Instructions to Offerors with the addendum that: "The offer submitted shall include: (i) Price Quote (Completed Attachment 2); (ii) Cage Code and TIN, and (iii) Completed FAR Clause 52.212-3,"Offeror Representations and Certifications-Commercial Items" or a statement indicating that they have been completed online at https://orca.bpn.gov. (2) FAR 52.212-2, Evaluation, with the following addendum for the basis of award: "The Government will award a contract resulting from this solicitation to the low priced technically acceptable vendor. To be considered technically acceptable, the vendor must be able to fully provide required specifications as stated in Attachments 1 and 2."
