DESCRIPTION This special notice for commercial items is posted as a Request for Quotation (RFQ) in accordance with Federal Acquisition Regulation (FAR) Part 12.203 and FAR Part 13.5 with consideration given to FAR Part 5.202(a)(13), as supplemented with additional information included within to provide: 0001 - PEDIATRICS REGISTERED NURSE - BASE YEAR PERSONAL SERVICES OF ONE PEDIATRICS REGISTERED NURSE FOR THE 90TH MEDICAL GROUP, FE WARREN AFB WYOMING, IN ACCORDANCE WITH THE ATTACHED POSITION DESCRIPTION AND PERFORMANCE WORK STATEMENT AND POSITION SUMMARY. PERIOD OF PERFORMANCE 1 OCTOBER 2011 THROUGH 30 SEPTEMBER 2012. 1001 - OPTION YEAR 1 PERSONAL SERVICES OF ONE PEDIATRICS REGISTERED NURSE FOR THE 90TH MEDICAL GROUP, FE WARREN AFB WYOMING, IN ACCORDANCE WITH THE ATTACHED POSITION DESCRIPTION AND PERFORMANCE WORK STATEMENT AND POSITION SUMMARY. PERIOD OF PERFORMANCE 1 OCTOBER 2014 THROUGH 30 SEPTEMBER 2015. 4001 - OPTION YEAR 4 PERSONAL SERVICES OF ONE PEDIATRICS REGISTERED NURSE FOR THE 90TH MEDICAL GROUP, FE WARREN AFB WYOMING, IN ACCORDANCE WITH THE ATTACHED POSITION DESCRIPTION AND PERFORMANCE WORK STATEMENT AND POSITION SUMMARY. PERIOD OF PERFORMANCE 1 OCTOBER 2015 THROUGH 30 SEPTEMBER 2016. Delivery will be to the 90th Medical Group, 6900 Alden Drive, FE Warren AFB, WY, 82005 The RFQ incorporates provisions and clauses that are in effect through Federal Acquisition Circulars (FACs) 2005-53. The proposed requirement is 100% Small Business Set Aside and applies to the North American Industrial Classification System Code (NAICS) 621399 with a size standard of $7,000,000.00. The evaluation criteria and relative weights to be utilized in order to award this requirement are as follows: quality of the healthcare worker, price, and past performance. The quality of the healthcare worker and price outweigh past performance as evaluation criteria. However, an integrated assessment will be made for a best-value determination for the Government. The contractor's proposal must include a price section and a technical section. The price section shall include the fully burdened hourly rate and the average hourly wage to be paid to the employee for each proposed health care worker. Technical proposal information will be streamlined, e.g. the technical proposal shall have two (2) sections-an Approach (limit two (2) pages) and Resumes (no page limit) of personnel that will be available for the MTF should an award be made. The "Approach" portion of your proposal shall not simply restate PWS/Position Summary requirements, but shall include your approach in fulfilling requirements. The Government reserves the right to award without discussions. If the Government decides to award with discussions, you will be notified as soon as possible and asked to provide candidate contact information. The Government will make a best value award decision IAW FAR Part 13.106-1(a)(2). Quotes shall include pricing information to include unit price and the extended total. To be eligible for contract award or payment from any DoD activity, offerors must be registered in the DoD Central Contract Registration (CCR) and Wide Area Work Flow Receipt and Acceptance (WAWF-RA) websites. Information on CCR registration and annual confirmation requirements may be obtained by calling 1-888-227-2423, or by registering via the CCR website at http://www.ccr.gov/. Information on WAWF-RA registration may be obtained by calling 1-866-618-5988, or by registering via the WAWF-RA website at https://wawf.eb.mil/. The WAWF-RA Invoicing System is now mandatory for use. There are no charges or fees to use WAWF-RA, and allows monitoring of your payment status online. Contractors must also submit their representations and certifications at the online Representations and Certification Applications (ORCA) website at http://orca.bpn.gov. There will be no paper copies of the RFQ notice available. The Government will not reimburse any interested parties for monies spent to provide a response to the subsequent RFQ notice. Responses to this notice must be submitted in writing within the time allowed.
