Description

(I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All SDVOSB submitting an offer must be registered and will be verified by the Contracting Officer in the VetBiz Information pages located at http://www.vip.vetbiz.gov/. All bidders must also be registered on www.ccr.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number VA-246-11-RQ-0975. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. (IV) This procurement is restricted to Service Disabled Veteran-Owned Small Businesses (SDVOSB). The associated NAICS code is 423450. The small business size standard is 100 employees. Vendors offering products other than the Audioscan Verifit Real Ear System shall demonstrate that their items perform all the sam functions, have the same dimensions and conform to the same construction and safety standards of the specified items. Wireless Bluetooth patient care modalities must be compatible with NOAHLink hearing aid programming systems. Systems not compatible with the current VA audiology-related modalities will be determined unsuitable for VA use. New equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's original warranty. CAUTION to OFFERORS. The purchasing activity is not responsible for locating or securing any information that is not identified in the RFQ or reasonably available to the purchasing activity. (VII) Required delivery of draft documentation within 30 days of site survey. Final documentation within 2 weeks after receipt of final review comments. Place of performances Durham VA Medical Center, 508 Fulton Street, Durham, North Carolina 27705. (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun, 2008), applies to this acquisition. (IX) FAR 52.212-2, Evaluation - Commercial Items (Jan, 1999), applies to this acquisition. Paragraph (a) is hereby completed as follows: the Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. Award will be made using the lowest prices technically acceptable process, where the non-price factors are evaluated on an acceptable/unacceptable or pass/fail basis. Award will be made to the offeror of the technically acceptable quote with the lowest price. (X) The offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (April 2011), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (XI) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items (Jun, 2010), applies to this acquisition. (XII) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Apr, 2011) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.211-6, Brand Name or Equal 52.219-27, Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (May, 2004), (15 U.S.C. 657f). Questions must be received no later than 12:00 Noon, SEPTEMBER 27, 2011. (XV) Quotes are required to be received by the Contract Specialist electronically via email no later than 2:00 P.M.
