Description
DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number VA-250-10-RP-0577 is being issued as Request for Quote (RFP) and the intent is to award a firm fixed price award for supplies. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This requirement is available as full and open competition for commercial items under the North Atlantic Industrial Classification System (NAICS) 517919 All Other Telecommunications; the business standard size is $25.0. REQUIREMENTS: The Dayton Veterans Affairs Medical Center (DVAMC), 4100 W. 3rd Street, Dayton, Ohio 45428, intends to negotiate a commercial item and is seeking Proposals for XM Satellite Radio/Signal Service in the new Women's Health Clinic. This equipment is to enhance the environment and care of our women veterans in the new Women's Health care center and serves as notification to the staff where their assistance is needed throughout the clinic. Enhances safety by knowing at a glance where someone is needed. STATEMENT OF WORK FOR XM SATELLITE RADIO/SIGNAL SERVICE 1. Services Primary Care Services, a clinic within The Department of Veterans Affairs (DVAMC), 4100 W. Third Street, Dayton, Ohio, 45428-9000 is also the parent facility for the Women's Health Clinic which is relocating to a new facility presently near completion and attached to building 340 Primary Care Services. Women's Health is a specialized clinic specific to the needs and care of women veterans. The atmosphere will be soothing and therapeutic which is conducive to this focused medical care. XM Satellite Radio and a wireless staff/patient notification system are required to accomplish this goal and for staff/patient safety needs. 2. General Requirements: 1. The company must be locally based within 150 miles for repair and/or maintenance purposes. 2. The company must be able to provide and install all necessary equipment for full operation other than AC Power, must be able to program, test/tune all satellite radio devices and wireless staff notification system to end user satisfaction. Others may supply conduit if needed. A.Requirements for Satellite Radio: Music source will be XM, which will be a separate purchase. The monthly fee for the service must also cover any FCC Regulations for playing music in a public place. 1.A Single Zone Satellite Radio with (1ea) Pixel XM Satellite Antenna and in- line amplifier is required. 2.(1 ea) 300 watt-70 volt 2 Channel Amplifier 3. 37 (each) 4 inch ceiling speakers with X-Former or 100 volt Distributed Line. 4.20 (each) Attenuator, 35 watts, 3Db (3 Decibel), 1 Gng Ss (1 Single Gang) 5.2 (each) 2 conductor 18 (7 x 26) Bare CMP (Communications, Plenum) 6.0.2 (each) RG-6 (Radio Guide 6 coaxial cable) 18 Solid Bare CATVP (Cable TV Plenum). B. Requirements for Wireless Staff/Patient Communication Terminal System: This system will basically function as a nurse call system, staff location information terminal, as well as a duplex intercom station. It will allow staff to know where a need is while identifying staff's location instantaneously. This system will also allow patient room communication. This should consist of 13 (each) terminals located in each room and 1(each) nurse console to be located at the nurses' station/front desk area. 1.13 (each) Staff Terminals should have 1 to 3 lines for call annunciation with the ability to provide information from incoming calls including staff/patient location, call priority and length of time call has been waiting. User must be able to selectively choose any call displayed. Each terminal should have a corridor light (13 each) to be mounted outside each room that when activated signals the presence of staff or patient at a glance. 2.Power supply with battery backup. 3.Must have optional hands free communication ability. 4.Emergency call upgrade must be available as well as Staff Follow, Sequential Room Monitor. 5.Each terminal should be able to have customized functions for its specific location and function. 6.Color touch screen display on the staff terminal. 7.Over 900 possible unique facility-defined call priorities. 8.Over 32 programmable functions. 9.Ability to dial rooms through touch pads on screens. PROVISIONS AND CLAUSES: This combined synopsis lists one or more clauses by reference with the same force and effect as if they were given in full text (FAR 52.252-2). The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, apply to this acquisition with the following addendum to the provisions of the standard provisions. In accordance with Federal Acquisition Regulation FAR 52.212-2 Evaluation-Commercial Items all offers will be considered for BEST VALUE. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers. (1) Technical capability. Additionally, any potential offerors quoting a brand equivalent must supply supporting documentation that the product proposal is of equal or greater technical capability matching the specified equipment in features, quality, warranty terms, compatibility with existing systems, and functionality to be considered. (2) Past performance: please include references of customers who have purchased a similar system. Past performance with VA hospitals or other Government agencies is preferred. Please include company name, point of contact information, purchase order or reference number, and dollar value. Those not submitting past performance references will be given a neutral rating. (3) Price: Proposal for each line item, including but not limited to the requirements listed above. Technical and past performances when combined are significantly more important than price. The provision at VARR 852-211-73, Brand Name or Equal applies. The items called for by this RFP have been identified in the schedule by a "brand name or equal" description, such identification is intended to be descriptive, but not restrictive, and is to indicate the quality and characteristics of products that will be satisfactory. Proposals offering "equal" products (including products of the brand name manufacturer other than the one described by brand name) will be considered for award if such products are clearly identified in the proposal and are determined by the Government to meet fully the salient characteristics requirements listed in the RFP. WARRANTY AND SERVICE: Indicate terms. DELIVERY AND INSTALLATION: F.O.B. Destination is preferred, please include any freight in the individual item pricing if applicable. The government will arrange for shipping and handling if the estimated cost for handling is over $250. Please include an estimated date of arrival. NOTE: The Government reserves the right to award without discussions, vendors shall submit the most favorable terms. Small businesses, especially service-disabled veteran owned small businesses and veteran owned are encouraged to contact their nearest Procurement Technical Assistance Center (PTAC) if assistance is needed in preparing their proposal. ELECTRONIC INVOICE SUBMISSION: The Department of Veterans Affairs (VA Financial Service Center (FSC) is the designated agency office for invoice receipt in accordance with the Prompt Payment Act (5 CFR part 1315). FSC or its designated representative may contact the vendor to provide specific instructions for electronic submission of invoices. The vendor will be responsible for any associated expenses. FSC may utilize third-party contractors to facilitate invoice processing. Prior to contact by FSC or its designated representative for electronic invoicing submissions, the vendor shall continue to submit all invoices to FSC at the following mailing address: Department of Veterans Affairs, Financial Services Center, and P.O. Box 149971, Austin, TX 78714-8971. Offerors shall complete FAR 52.212-3 Offerors, Representations and Certifications-Commercial Items available for online registration at http://orca.bpn.gov or include a written copy with the proposal submitted.
