- Statement of Work This posting solicates proposals for Quarterly Health Assessments at two locations, Washington DC and Rosslyn VA. The assessments will each be in three parts, with the first covering bloodwork, the second fitness testing and the third providing a printed report for use in counseling individual members (the report will be given to the member). Members who decline the counseling session will have the option of having the report mailed to them. In addition to this quarterly fitness assessment, an annual fitness fair to be held in the August/September timeframe is to take place at both locations (not simultaneously). Subjects should include but are not limited to Glaucoma testing, lung capacity, percentage of body fat, blood pressure, osteoperosis testing, and skin damage testing. Interested parties should read the attached Statement of Work and provide costs per individual for CLINS 001-003. Provide a price per seminar for CLIN 004. For CLIN 005 provide a total price for the Health Fair.
