- THIS NOTICE OF INTENT IS BEING MODIFIED TO CHANGE THE NAICS CODE. THE CHANGE HAS BEEN BOLDED. THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The National Heart, Lung, and Blood Institute (NHLBI) Office of Acquisition (OA) intends to negotiate and award a purchase order on a noncompetitive sole source basis to Wisconsin University, 475 N. Charter Street, Madison, Wisconsin 53706 to conduct a web to mail mixed-mode survey with three samples of 800 (total 2400 individuals). The sole source determination is based on the fact that National Institutes of Health (NIH) is the nation's leading medical research agency and the primary Federal agency conducting and supporting medical discoveries that improve people's health and save lives. The Bioethics Core and Bonham Lab with the National Human Genome Research Institute (NHGRI) are continuing an ongoing collaborated research partnership with Wisconsin University, and will collaborate to work on a new study to understand attitudes of patients, research participants and blood donor for incidental findings from genomic information received by the participation in genetic and genomic clinical care, research studies and blood donations services. Wisconsin University will support both laboratories by conducting a web to mail mixed mode survey of samples of 800 (2400 individuals). Due to Wisconsin's University experience with survey administration and research collaboration to advance biomedical, social science, and bioethics research, as well as previous experience and collaboration with NHGRI that is why this requirement is sole sourced to Wisconsin University. REQUIREMENT : Specific Tasks : Task 1: Provide expert advice on survey development • The contractor will assist the Bonham and Hull Labs in survey development to maximize the quality of the data obtained and the burden of the survey on the respondent. Task 2: Survey Web-based Programming Development • The contractor will program and debug the survey for the web and prepare the printed survey instrument. The contractor will work with the NHGRI graphics staff to assist in the professional web-based survey for the user. Task 3: Mail invitation and email reminders • The contractor will collaborate with the Bonham and Hull lab in the development of recruitment materials including invitation letter and email reminders. The contractor will send and initial mailing first class USPS mail and three email reminders to each individual in the survey sample. He/she will also track who has completed the survey and determine which e-mail addresses are invalid for subsequent contacts. REQUIRED REPORTS/DELIVERABLES: • Contractor will deliver a cleaned dataset provided in SPSS, SAS and STATA along with appropriate documentation. • Contractor shall provide codebook with percentages and absolute number frequency distributions of every coded variable • Contractor shall provide responses to all open ended questions • Contractor shall provide response rate report and documentation procedures as agreed upon with the investigators Industry Classification (NAICS) Code is 541990, All Other Professiona, Scientific and Technical Services, with size standard of $7.0M. The acquisition is being conducted under FAR Part 13, simplified acquisition procedures, therefore the requirements of FAR Part 6 B Competitive Requirements are not applicable (FAR Part 6.001).and the resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular (FAC) 05-53 (August 4, 2011). This notice of intent is not a request for competitive proposals. Interested parties may identify their interest and capabilities in response to this synopsis, by September 16, 2011, 7:30 a.m. Eastern Standard Time. The determination by the Government not to compete the proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct future competitive procurement.
