Description

Notice to Contractor: Solicitation number N00259-12-T-0011 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is for Artificial Limb Specialist is being utilized to perform patients gait/ range of emotion and to restore a manner of normal gait for our C-5 Patients. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is Artificial Limb Specialist, 2916 North 3rd Street, Phoenix AZ 85012, CAGE Code: 3842, NAICS: 339113. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that Artificial Limb Specialist is the only company that manufactures and distributes the Prosthetics devices. This product led to be the best results to the patient medical care and it ™s cost effective. This equipment is the only compatible with the existing machines and is currently owned machines at the NMCSD. IAW FAR 5.207(c) (15) (ii), all responsible sources may submit a quotation, which shall be considered by the agency. The government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-44. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil.
