- Scope of Work Standard Form 1449 & Business Management Questionnaire Financial Management U.S. Department of Justice Federal Bureau of Prisons Federal Correctional Complex Terre Haute, Indiana Terre Haute, In 47802 October 19, 2011 Re: RFQP04181200004, Water Testing Inspection Federal Bureau of Prisons Federal Correctional Complex, Terre Haute, Indiana Dear Quoter: This cover letter precedes a solicitation package prepared by the Federal Bureau of Prisons, Federal Correctional Complex, for the acquisition of Water Testing Inspection to be provided for the Federal Correctional Complex located in Terre Haute, Indiana. This package contains all necessary information for submitting a quote for evaluation. When submitting quotes, quoters should consider all information provided herein. Please carefully follow all instructions located in the solicitation package concerning the content, format, and submission of quotes. The following areas need to be completed and returned: Standard Form 1449 Offeror Representations and Certifications Business Management Questionnaire This solicitation is being issued as a total small business set-aside. Hard copies of the solicitation will not be available. All contractors doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov. Faith-based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. All responsible small business concerns are encouraged to submit a quotation which may be considered. All information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. All potential quoters are advised that this solicitation includes the clause FAR 52.212-1, Instructions to Offers-Commercial items which stipulates in part inclusion of a DUNS number with your offer and registration in the Central Contractor Registration (CCR) database located at www.ccr.gov. Failure to comply with the instructions detailed in this provision may result in non-consideration of your quote. Sealed quotes shall be received on or before the deadline of 4:00 P.M. Eastern standard time, Monday, October 24, 2011. Quotes shall be submitted to the following address: Federal Bureau of Prisons Federal Correctional Complex-USP Attention: Tammy Stewart, Contract Specialist 4700 Bureau Road South Terre Haute, In. 47802 Mailed and hand-carried quotes must be delivered to the Contract Specialist at the address shown above no later the date specified as the deadline for receipt of quotes. The official point of receipt for hand-carried quotes will be the USP Front Entrance. PLEASE DO NOT SUBMIT QUOTES TO ANY LOCATION OTHER THAN THE ONE LISTED ABOVE. Quotes received after the deadline will be handled in accordance with FAR provision 52.212-1(f). Faxed and emailed quotes will not be accepted. Please place questions you may have regarding solicitation in writing and fax them to 812-244-4573 to my attention or e-mail them to me at tstewart@bop.gov.
