Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is N40442-12-T-7061, and it is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53, effective 5 JULY 2011. NAICS code 336611 applies. This is a small business set-aside. Military Sealift Fleet Support Command (MSFSC), Norfolk, VA intends to award a firm fixed price purchase order for a qualified and approved Allied Systems Technical Representative for USNS MERCY for the following services: The requested period of performance for the below service is 06 - 15 DEC 2011. 1.0 ABSTRACT 1.1 This item provides for the replacement of existing deteriorated decking with Terrazzo coverings in designated toilet and shower spaces, to replace cracked and deteriorated shower enclosure joiner panels and deteriorated stateroom head panels. 2.0 REFERENCES/ENCLOSURES 2.1 References: 2.1.1 MSC DWG 5981322, Deck Covering Schedule (available onboard). 2.1.2 NSTM Chapter 634, Deck Coverings. 2.1.3 MSC DWG 5980824_D Composite Arrangement, 1st, 2nd & 3rd Platform T/S Spaces 2.1.4 MSC DWG 5981318_B Joiner BLKHDS & Sheathing Typical Details 2.1.5 MSC DWG 5981347_E Shower Area Details 2.2 Enclosures: None. 3.0 ITEM LOCATION/QUANTITY 3.1 Location/Description 3.1.1 Locations: Eight (08) Aft House stateroom heads 3.1.2 Approximate 25 Sq/Ft each 4.0 GOVERNMENT FURNISHED EQUIPMENT/MATERIAL/SERVICES: None 5.0 NOTES 5.1 All sizes, dimensions, and quantities provided are approximate. The contractor shall verify all sizes, dimensions, and quantities prior to commencement of repairs. 5.2 All deck covering and bulkhead panel colors shall be approved in advance by the MSCREP. 6.0 QUALITY ASSURANCE REQUIREMENTS 6.1 All work performed shall be to the satisfaction of the MSCREP. 7.0 STATEMENT OF WORK REQUIRED: 7.1 Cover stateroom and passageway decks with herculite or other suitable material prior to commencement of repairs to prevent contamination. Stateroom furnishings shall also be covered securely with heavy duty plastic prior to commencement of repairs to prevent contamination. 7.2 Remove and reinstall interferences as necessary to gain access to repairs. 7.3 Replacement deck covering colors shall be approved in advance by the MSCREP. 7.4 Replace existing deck covering, coamings, and underlayment/insulation in the locations specified in paragraphs 3.1 in accordance with references 2.1. 7.4.1 Crop, remove and dispose of existing deck covering, coamings, and underlayment/ insulation in their entirety. 7.4.2 Mechanically clean exposed steel deck and bulkhead surfaces to bare metal and insure that all vestiges of existing deck coverings are removed to expose bare steel. 7.4.3 Make repairs to steel coamings in way of leaking valves and paint disturbed surfaces in locations in para 3.1.1. For bidding purposes figure 48 feet of coaming repairs. 7.4.4 Conduct a visual inspection of the steel decking in the presence of the MSCREP. Checkpoint. 7.4.5 Apply two (2) coats of epoxy primer to the bare metal surfaces. 7.1.6 Install new underlayment, insulation, Terrazzo deck covering, and coamings in accordance with references 2.1, and the manufacturer's specific instructions, insuring proper slope for drainage. 7.4.6 New decking shall extend up approx. 6" around the perimeter of shower enclosures and perimeter bulkheads and shall be tapered from top to bottom. Top of coamings shall be sealed with clear silicone caulk to ensure water tightness. 7.4.7 Apply a minimum of two (2) coats of sealer to the new decking. 7.5 Replace existing shower enclosure panels in the locations specified in paragraphs 3.1 in accordance with references 2.1. NOTE there are two steel panels and two joiner panels for each shower. Thirty-two (32) panels total. 7.5.1 Remove and dispose of existing shower enclosure joiner panels. NOTE there are Four (4) panels for each shower. Thirty-two (32) Joiner Panels total. 7.5.2 Provide and install new CFM shower panels. 7.5.3 Fabricate and install eight (08) water-tight access panel to facilitate access to shower valves and toilets in the locations specified in paragraphs 3.1. 7.5.4 Provide and install new Stainless steel shower racks, soap holders and towel racks in each head and shower approved by MSCREP. 7.6 Provide and replace new CFM Head Panels that are identified during the replacement of decking and shower joiner panels that are found as "deteriorated". The actual panels to be replaced shall be with approval the MSCREP. For bidding purposes provide 10 Head Panels. 7.7 Mechanically clean, prepare, prime and paint all new and disturbed areas identified in para 7.4, 7.5 and 7.6 to match surrounding surfaces. 7.8 Daily remove all dirt and debris generated by this item and shall be removed from the ship daily and all work areas left clean and in good order. 7.9 Upon completion of work there shall be an official turnover of each space and acceptance of MSCREP. MSFSC INVOICE INSTRUCTIONS FOR SHIPPING CHARGES At the time of invoice, a shipping receipt must be provided to substantiate shipping or freight charges greater than $25.00. BASIS FOR AWARD: Award will be made to the responsible, technically acceptable quoter, whose quote, conforming to the combined Synopsis/Solicitation, offers the lowest evaluated price. SHIP & BASE ACCESS Vendors are responsible for correctly submitting forms/applications. Vendors are encouraged to monitor email using "Request a Read Receipt" function and to confirm receipt of facsimile transmissions. Due to enhanced security measures, ship and base access is required for pre-award ship visits (e.g. ship-check) and for contract performance. Specifically the following permissions are required: 1. El Paso Intelligence Center (EPIC) personnel screening requirement 2. Base/Repair Facility Access Request 3. Vehicle Access Request 4. EPIC personnel screening requests are desired seven calendar days prior to performance start date. Requests will be managed as expeditiously as circumstances permit. Vendor will be notified by MSFSC of personnel who are denied access to the vessel. Base/Repair Facility Access: Permit is required to access Navy facilities. Base/Repair Facility Access Request is desired seven days prior to ship check or performance start date. Submit forms per the document instruction and to the Port Engineer. A courtesy copy (CONUS West Coast performance locations only) to Ermanno Magliulo, Engineering Director, at ermanno.magliulo@navy.mil will be appreciated. RAPIDGate® is being implemented for access to DoD facilities in CONUS, HI, Guam and Puerto Rico. Vendor is responsible to acquire RAPIDGate® status and destination base individual access badge(s) activation prior to performance start date. Vendor is responsible to confirm that each employee held annual RAPIDGate® badge is active for the specific facility and performance period in accordance with RAPIDGate® User Agreement (http://www.RAPIDGate.com). Additional access permissions may be required such as Navy Base Point Loma "Enclave Access request form". Vehicle Access: Required for vehicle access to Navy facilities. Follow supplemental instructions on Base Access forms or base-specific vehicle access forms. Ship Access List* (Vendor-Provided): On company letterhead attachment via email, the vendor is required to provide the ship master (courtesy copy the Port Engineer) with an accurate, current list of performing personnel prior to being admitted aboard the vessel. Under no circumstances will a hand-delivered list be accepted.
