Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. Solicitation No. _____________ herein cited will be procured under FAR Part 12, Acquisition of Commercial Items. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This acquisition is a full and open competition, NAICS Code: 424490. DESCRIPTION: The contractor shall provide services consisting of furnishing all labor, materials, equipment, supervision, transportation and incidentals necessary to deliver bread goods weekly from date of award through September 30, 2012, orders will made by authorized Government personnel only and in accordance with FAR Clause 52.232-18, Availability of Funds (April 1984). Delivery requests will be on a weekly basis by the authorized personnel listed below. All bread products shall be fresh at the time of delivery and shall be inspected and accepted by the Government staff and notation of any damaged items. All damaged items shall be replaced at no additional cost to the Government. Deliveries shall be requested weekly by the authorized individuals, and invoice weekly. All interest offerors shall provide inclusive of all costs for the requirement; including but not limited to all applicable taxes and FOB shipping cost. (All bread products must be fresh, frozen products will not be accepted). The following terms, conditions, provisions, and clauses are applicable to this acquisition: FAR 52.212-1, Instructions to Offerors-Commercial Acquisition, and FAR 52.212-2 Evaluation Commercial Items;FAR clause 52.212-4, Contract Terms and Conditions, - Commercial Items; FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Order-Commercial Items; FAR clause 52.204-6, Contractor Identification Number Data Universal Numbering System (DUNS) Number; FAR 52.232-34, Information For Electronic Funds Transfer Payment other than CCR. The Government reserves the right to make award on the initial proposal received without discussions. Information must be received before 4:00 PM, EST, 24 March 2006 Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The anticipated award date is on or before 31 March 2006. Please direct all questions regarding this procurement to Brenda Espinueva, Contract Specialist by email at Brenda.Espinueva@va.gov only. Your proposal shall include the company name, address, telephone number, DUNS number, Tax Identification Number. Quote of the items being offered in detail to evaluate compliance with requirements in the solicitation price per each, total price, state business size in a completed copy of Representations and Certifications-- Commercial Items, and an acknowledgement of all amendments. The offeror agrees to hold the prices submitted in the proposal firm for 60 days from the date and time specified for receipt of proposals. The offeror shall submit a statement of agreement with the terms and conditions included in this Solicitation. As part of the quote, each offeror shall provide past performance information for the same or similar items for three of the most recent contracts and include the following information: Contact person and company, telephone number, Contract number, Dollar value of the contract, and the product provided. Offers that fail to furnish required documentation or reject the acquisition terms and conditions may be excluded from consideration.
