POC: NORIKO YAMADA noriko.yamada.ja@usmc.mil FOB: Destination MILSTRIP: M0220912RC00008 PURCHASE REQUEST NUMBER: M0220912RC00008 NET AMT DESCRIPTION/SPECIFICATIONS/WORK STATEMENTS Statement of Work for Supervisory Education Director “ MCAS Iwakuni SECTION I DESCRIPTION OF SERVICES 1.GENERAL DESCRIPTION 1.1OBJECTIVE: The key objective is for a station wide Christian non-denominational religious youth ministries program, established and conducted for middle and high school students, that supports specific youth ministry needs and other established programmed services of the Marine Memorial Chapel, Marine Corps Air Station (MCAS) Iwakuni Japan to include programming, development and implementation of youth activities, and general Religious Education. 1.2SCOPE OF WORK / CONTRACTOR REQUIREMENTS: Non-personal services. The Contractor shall provide personnel to establish, implement and maintain a strategy, and comprehensive plan for a Youth Ministry Program for the Marine Memorial Chapel population (estimated at 4000 personnel with 150 high school/middle school youth age) located on MCAS Iwakuni, Japan. Contractor Personnel shall perform the specific tasks in accordance with this Performance Work Statement. 1.3SPECIFIC TASKS: The Contractor shall provide and coordinate chapel and chapel related youth ministry activities for middle and high school students. The Contractor shall coordinate with local schools, churches, and youth agencies to ensure overall coordination of MCAS Iwakuni youth ministry activities do not conflict with other youth activities. The Contractor shall provide a station wide Christian religious program, supporting specific youth ministry and parish needs in addition to those provided by Marine Memorial Chapel staff and volunteers. The Contractor shall articulate and develop promotional campaigns and endeavors, maintain records of supplies, materials, and attendance to include youth, volunteers, organizations, and agencies. 1.3.1The following programs and support requirements are considered minimal elements of contractor ministry activity services. 1.3.1.1High School Program (Grades 9 “ 12) a.Youth Club Meetings - Organize and direct weekly regular high school youth outreach club meetings that integrate mental, physical, social and spiritual development. Contractor to conduct at least 48 Club Meetings a year. b.Bible Study - Organize and conduct weekly Bible study or small group activities for high school youth that focuses on the study of scripture and involves discussion topics or relational issues that expand on the knowledge of the Bible or spiritual growth. c.Weekend Retreat - Conduct two or more high school youth outreach retreats each year to provide students to experience growth in their faith and in the areas of services or discipleship. The Contractor shall submit a proposal plan to include objectives, location(s), dates, times, budget, sponsor requirements, method of evaluation effectiveness no less than 90 days prior to the event. d.Extended Mission - Conduct at least one extended (5-7 days) service mission for high school youth each year to provide youth the opportunity to gather for fun, fellowship and faith sharing. The Contractor shall submit a proposal plan to include objectives, location(s), dates, times, budget, sponsor requirements, method of evaluation effectiveness no less than 90 days prior to the event. e.Special Events “ Conduct four special events (two per school semester) to promote group growth, nurture or discipleship with the goal of providing team building, community responsibility and faith sharing. (e.g. community service projects, fund-raising, lock-ins, etc) f.Visitations “ Develop supportive relationships with air station/base youth and high school(s) faculty through visitation to High School(s) locations, with no less than 72 occasions during the school year, to build relationships through the involvement in school programs and providing a strong Christian role model in the school setting. g.Mentoring “ Mentor youth in the areas of spirituality, outreach, Christian growth, morals and values throughout the year, which may include parental interaction. 1.3.1.2 Middle School Program (Grades 6 “ 8) a.Youth Club Meetings - Organize and direct regular middle school youth outreach club meetings that integrate mental, physical, social and spiritual development. Contractor to conduct at least 48 Club Meetings a year. b.Bible Study - Organize and conduct weekly Bible study or small group activities for middle school youth that focuses on the study of scripture and involves discussion topics or relational issues that expand on the knowledge of the Bible or spiritual growth. c.Weekend Retreat - Conduct two or more middle school youth outreach retreats each year to provide students to experience growth in their faith and in the areas of services or discipleship. (e.g. community service projects, fund-raising, lock-ins, etc) e.Visitations “ Develop supportive relationships with air station/base youth and high school(s) faculty through visitation to Middle School(s) locations, with no less than 72 occasions during the school year, to build relationships through the involvement in school programs and providing a strong Christian role model in the school setting. f.Mentoring “ Mentor youth in the areas of spirituality, outreach, Christian growth, morals and values throughout the year, which may include parental interaction. 1.3.1.3 Ministry Activities and Reporting a.Service Project “ Provide a service project for a ministry year calendar, to forecast ministry resources requirements and project ministry activities. The Contractor shall include a plan to conduct and execute a minimum of 40 youth outreach events per year emphasizing relationship building, social interaction, and recreation. b.Proposal Plan Records - For all Retreats, Missions, and Special Events: Maintain a record of all proposal plans that outline objectives, locations(s), dates, times, budget, sponsor requirements, method of evaluating effectiveness, number of volunteers utilized and number of persons attending each event. Each record shall also include the permission slips, power of attorney, etc as required for off post events. c.After Action Reporting “ Provide a written after-action report for all Retreats, Missions and Special Events to include suggested changes, direction, trends, and problem areas. After Action Reports shall be due to the COR within two weeks of event completion. d.Community Relations - Develop supportive relationships with youth; maintain close liaison with chaplains, the command and schools. Develop supportive relationships with other community agencies of MCAS Iwakuni, Japan. 1.3.1.4 Volunteer Activities a.Recruiting - Recruit and train adult volunteers on a weekly basis to assist in the conduct of the ministry program. The Contractor shall be responsible for ensuring that volunteers recruited to assist in youth activities is consistent with religious teachings within the chapel. Selection of volunteers will be in accordance with procedures established by the contractor and approved by the Command Chaplain. Volunteers shall be subject to completion of background checks. The contractor shall be provided a quota on the number of volunteers needed to meet ministry program requirements. b.Mentoring and Training “ Conduct ongoing monthly training sessions to mentor and train volunteers in all aspects of youth ministry with the goal of establishing an effective volunteer by teaching skills and confidence in dealing with youth ministry. c.Volunteer Roster - Maintain and update a volunteer roster to include name, address, phone number, military affiliation, utilization and role, training and hours volunteered to be accessible by the Command Chaplain. 1.3.1.5 Administration a.Forecasting “ Based on the Contractors Service Project, forecast ministry resources and budget requirements for projected ministry activities, on a quarterly basis, to include the procurement of supplies, literature and curriculum needed for the program. The Contractor shall be responsible for the submission of necessary paperwork for scheduling of facilities in support of programs and the arrangement of audiovisual equipment as needed. It is estimated that the annual budget range for program use will be approximately $6,000.00 to $7,000.00 per year. b.Monthly Reporting - Submit monthly reports to the COR or designee within one (1) week after the months end, outlining and evaluating ministry activities and plans and keeping the chaplain informed of the status of the programs, future scheduling and any problems that may arise. c.Annual Reporting “ Provide an annual review of the youth ministry program to the Command Chaplain during the month of September summarizing the year ™s activities, youth ministry trends, progress and suggestions for improvement regarding future program direction. d.Advertising and Promotion - Draft and coordinate advertising, promotional bulletins announcements, articles, flyers and materials in a timely manner for all youth activities and events for publication and distribution. Publication and distribution can include but are not limited to the air station/base chapel congregations, newspapers, fliers and AFN television and radio The use of free advertising will be employed as much as possible. Costs incurred for advertising shall be pre-approved by the Chaplains Office for available funding. e.Briefs and Meetings - Maintain communication and cooperation with all personnel and parties involved in the youth ministry program and community. Such activities include conducting and/or attending Parent Meetings, Command Chaplain Staff Meetings/ Appointments and Chapel/Parish Councils. f.Office Hours “ Spend an average of 8 hours per week (twice a week) in the chapel office, maintaining face to face interaction and report with the Protestant chaplains. 1.3.1.6 Bi-Annual Education Training Spearhead and organize a bi-annual (Fall/Spring) teacher ™s/volunteer ™s training for all the education volunteers. This training needs to be an extensive ½ day training that deals with essential educational topics dealing with teaching small groups and serving as a volunteer and mentoring. 1.4PERIOD OF PERFORMANCE: 01 December 2011 through 30 September 2012. 1.5. LOCATION. The primary facilities for the services provided will be the Marine Memorial Chapel, MCAS Iwakuni Japan. The contractor, upon assignment by the Contracting Officer's Representative (COR), shall provide services in any chapel or approved facility located within the confines of MCAS Iwakuni, Japan. No variation. SECTION III GOVERNMENT-FURNISHED PROPERTY AND SERVICES 3.1 FACILITIES AND EQUIPMENT: The Government will provide the following facilities and equipment at no cost to the Contractor. Facilities and equipment shall not be assigned to the Contractor on a permanent basis and shall be used as needed for performance of services under this contract. 3.1.1Office Space: Office space shall including a desk, chair, filing cabinets, and related support materials. The Contractor will be given access to a telephone and fax machine with local phone service. 3.1.2Meeting Facility: A meeting facility to hold activity/Religious Education meetings as part of the ministry. 3.1.3Classroom: Classrooms with chairs, teacher ™s desk, white boards, etc, for performance of this contract. These properties will not be assigned permanently to the Youth Ministry Director. 3.1.4Equipment: Use of an overhead projector, TV, VCR, DVD player, laptop computer, projector, screen, use of copy machine and lawful reproduction privileges, use of videos for teaching and programming as available in the chapel resource center. 3.1.5Contractor Furnished Personal Property: The contractor shall ensure that personal property is clearly marked and identified to preclude misidentification as Government property. The Government will not be liable for loss, damage, theft, or maintenance of personal property. 3.2 SERVICES: The Government will provide reasonable amounts of the following services: 3.2.1 Transportation: Transportation requirements necessary in performance of the contract will be coordinated by the COR or designated representative and approved by Air Station/ Base Motor Transport. 3.2.2 Utilities: The Government will provide all utilities, such as water, sewage service, electricity, heating and air conditioning, to perform the services required by the contract, at the site of delivery of the services on federal property. The Government will not make any modifications to utility outlets to accommodate Contractor equipment. 3.2.3 Police and Fire Protection: The following telephone numbers are furnished: Emergency119 Provost Marshall253-3933 Fire Protection119 SECTION IV GENERAL INFORMATION 4.1 QUALITY ASSURANCE: The Government will monitor the Contractor ™s performance through its Quality Assurance Surveillance Plan in conjunction with the Contractors Quality Control Plan. The Government may increase or decrease the number of quality assurance inspections as performance dictates. The Government reserves the right to inspect the Contractor ™s performance to ensure services are received and in compliance with the terms and conditions of the contract. Performance meetings may be held regularly or as requested by the Contracting Officer, COR, or Contractor. 4.1.1 Performance Evaluation Meetings: The Contractor shall meet with the Installation Chaplain once per month. The Contractor shall contact the Installation Chaplain to make appointments for this meeting. The purpose of these meetings will be to discuss the needs assessment of the religious population located at MCAS Iwakuni, program evaluation and program/activity scheduling. The COR shall notify the Contracting Officer of any discrepancies, disputes, or indications of poor performance. 4.1.2 The COR will coordinate with the Contractor all matters that arise in the administration of this contract. In addition, the COR will monitor the Contractor ™s progress and performance to assure compliance with the requirements of the contract in accordance with the stated contract terms. The COR will serve as the representative of the Contracting Officer on all matters involving quality assurance. Should a quality or performance-related dispute arise, the Contracting Officer will serve as the final determining official. 4.2 QUALITY CONTROL: The Contractor shall develop, and maintain a Quality Control Plan (QCP) to ensure required services are performed in accordance with commonly accepted commercial practices. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor shall provide a copy of the QCP to the Contracting Officer no later than the pre-performance conference, and updated copies as changes occur. Records of all inspections conducted by the Contractor and corrective action taken shall be made available to the Government upon request. 4.3 GENERAL DEFINITIONS 4.3.1Contracting Officer (CO) - US Government official authorized to enter into, make changes to, or terminate contracts on behalf of the US Government. Contracting Officers are responsible for ensuring performance of all necessary actions for effective contracting, ensuring compliance with the terms of the contract, and safeguarding the interests of the United States in its contractual relationships. 4.3.2 Contracting Officer Representative (COR) - Government liaison appointed in writing by the Contracting Officer and designated in the contract, who functions as the representative monitoring cost/price, delivery and performance of the Contractor. The COR is not authorized to enter into any agreements, obligate the Government or direct any changes to the contract. 4.3.3 Government Furnished Property (GFP) “ Property in the possession of, or directly acquired by, the Government and subsequently made available to the Contractor. 4.3.4Quality Assurance (QA) - Various functions, including inspection, performed by the Government to determine whether a contractor has fulfilled the contract obligations pertaining to quality and quantity. 4.3.5Quality Control (QC) - A complete Quality Control outline that meets the requirements of the contract shall be provided by the Contractor to illustrate how the company will ensure they conform to the contract requirements. 4.3.6Ministry Activities - Those responsibilities and activities that relate to the celebration of the Mass and worship opportunities and community building among the religious population of MCAS Iwakuni, Japan. 4.4SECURITY REQUIREMENTS 4.4.1 Physical Security: Contractor shall be responsible for safeguarding all government property provided for Contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured, and utilities turned off or set for conservation. The Contractor shall ensure that all facilities used are properly secured upon departure following the conclusion of a program or event when a chaplain or chaplain service support personnel is not present for duty. The Contractor is responsible for knowledge of and compliance with all chapel policies regarding maintenance, use and scheduling of facilities and chapel resources. 4.4.2 Identification and Base Passes: The Contractor shall obtain identification cards and base passes for all employees and vehicles. Application shall be coordinated through the Contracting Officer or designated representative. At time of application, Provost Marshall may conduct a background check on employees. Upon termination of employment, the contractor shall return identification cards and base passes to Provost Marshall within 24 hours. 4.4.3 Key Control: The Contractor shall establish and implement methods of ensuring that all keys issued to the Contractor by the Government are not lost or misplaced, and are not used by unauthorized persons. The Contractor shall report lost keys to the COR within 24 hours of discovery of the occurrence. The Contractor shall not duplicate any keys 4.4.3.1 The Government will require the Contractor to replace keys, reimburse the Government for replacement of locks, or re-key as a result of Contractor losing keys. In the event a master key is lost or duplicated, the Contractor shall replace all locks and keys for that system and the total cost will be deducted from the monthly payment 4.4.4Damage To Government Property And Facilities: In instances where Government Property or Facilities have been damaged by the Contractor, a full report of the facts and extent of damages shall be submitted to the COR within 24 hours following the incident. The COR shall forward the report to the Contracting Officer within 24 hours of receipt. If the Contractor is found to be negligent, the Contractor shall be held responsible for all expenses related to reimburse of all damages to the Government. 4.4.5Safety: It is the Contractor ™s responsibility to ensure the safety of their personnel. Requirements for safety will be strictly adhered to as they pertain to Contractor operations and as required. The Contractor shall implement safety practices in accordance with Station and Chapel policies. The Contractor shall enroll in any all hands safety related training provided by the Air Station/Base. 4.5CONTRACTOR PERSONNEL: 4.5.1 Contract Manager: The Contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate, who shall act on behalf of the contractor when the manager is absent, shall be designated in writing to the Contracting Officer. 4.5.4.1The contract manager shall have full authority to act on behalf of the contractor on all contract matters relating to the daily operation of this contract. 4.5.4.2Points of Contact. The contractor shall provide the Contracting Officer and the COR with the names of a primary and alternate point of contact with telephone numbers, in case of emergency. 4.5.2 Standards Of Dress: The contractor ™s employees shall present a neat well-groomed appearance appropriate for chapel worship services during all times while he/she is performing services under this contract. Cut-offs, pedal pushers, shorts, or tank/halter tops are not acceptable for office wear, but acceptable for appropriate activities. Any hair length, beards, or mustaches are acceptable, provided it is neat, clean and properly trimmed. Contractor employees conduct shall be professional in manner. 4.5.3 Contract Personnel Measures: 4.5.3.1Competency. All services under this contract are to be performed by an adult. Contractor personnel shall be able to read, write and speak English well enough to effectively communicate with youth and co-workers. 4.5.3.2Conduct: Contractor personnel shall exercise discretion regarding persons counseled or interviewed and any other information relating to individuals participating in the Marine Memorial Chapel Ministry program. The Contractor shall not relate or provide sensitive information regarding such individuals nor shall the Contractor provide originals or copies of personal notes, checklists, or records regarding such individuals to any other persons without express approval from the Installation Chaplain, or other competent authority. This duty of discretion shall in no way be construed as providing Contractor personnel the power or œpriest-penitent  privilege as provided in law. 4.5.3.3Attendance: The contractor shall ensure that adult supervision/leadership is present not later than 10 minutes prior to the start time of the scheduled activities, and remains present until the last participant under the age of 18 has departed the location. 4.5.3.4Extended Periods Of Absence. In the absence of the Contract Manager, the Contractor shall provide adequate coordination (15 days notice) and identify to the Installation Chaplain an alternate individual to assume the duties of the Contract Manager. Alternate persons fulfilling Contract Manager duties must be familiar with current Marine Memorial Chapel programs and activities and receive approval from the Installation Chaplain prior to performing in the absence of primary Contract Manager. The contractors alternate Contract Manager shall be a qualified substitute in the event the of primary Contract Manager absence. A "qualified substitute" is defined as an individual who meets the qualifications outlined in paragraph 4.5.4. 4.5.4Contractor Personnel Qualifications: 4.5.4.1General: Contractor Personnel shall be qualified in youth ministry as a result of previous training, education and experience and shall be expected to maintain his/her skills through ongoing upgrade training. This shall be completed at opportune times in the calendar year so as to minimize any potential conflict with existing youth ministry requirements. 4.5.4.2Ministry in the Military Environment: The challenges and struggles of military teens are similar to those of the typical teenager, but intensified because of their situation. The Contractor shall provide references from all previous ministry experiences as required for Contractor Personnel. 4.5.5Background Checks: Federally mandated background checks will be performed in accordance with Public Law 101-647, Section 231 (Crime Control Act of 1990) and Department of Defense Instruction 1402.5 (dtd 19 Jun 93), Criminal History Background Checks on Individuals in Child Care Services Positions, on all individuals working with young adults in conjunction with this contract. The Contractor shall provide to the COR all necessary information and forms required on individuals for the performance of background checks prior to personnel assigned duties under this contract. 4.5.6. Removal Of Contractor ™s Employees: The Contractor agrees to utilize only experienced, responsible and capable people in the performance of the work. The Contacting Officer may require that the contractor remove from the Government job employees who endanger persons or property, whose continued employment under this contract is inconsistent with the interest of military security, or who are determined by the Contracting Officer or COR as being incapable of rendering the services in accordance with this Performance Work Statement. 4.5.7Passes For The Contractor ™s Employees: The Contractor shall submit a request for base passes for his employees to the Marine Corps Regional Contracting Office, Okinawa, for approval by the Contracting Officer. Upon approval, the Contractor shall hand-carry the request to the Pass and Identification Section, MCAS Iwakuni. The Contractor shall be responsible for all passes issued to him or his subcontractor ™s employees under this contract. The contractor shall return all applicable base passes upon termination of employment or completion of this contract to the Pass and Identification Section and certification of receipt shall be obtained. The contractor shall submit lists of all employees terminated to the Contracting Office within seven (7) days of the effective date. 4.5.8Benefits: This is a Non-Personal Service Contract. No provisions are implied or provided in this contract for any benefits such as medical care, retirement, or worker ™s compensation. 4.5.9 Status Of Forces Agreement (SOFA). The Contractor shall be responsible for ensuring personnel performing under this contract obtain the necessary exit/entry visa permits to perform work in the Country of Japan. Upon request, the Contractor shall provide proof of status by presenting visa documents. 4.5.9.1Contractor Personnel who are U.S. citizens may apply for Status of Forces Agreement (SOFA) status as necessary for the execution of this contract. The determination of SOFA Status is processed by the Office of the Staff Judge Advocate, Marine Corps Air Station Iwakuni to COMUSJAPAN. The Contractor shall be responsible for submitting necessary paperwork to the Office of the Staff Judge Advocate for determination of designation of SOFA status. 4.5.9.2Contractor employees will be subject to all U.S. Forces regulations and directives pertaining to civilian component personnel within the country. Privileges are further limited to the following: Entry into and exit from Japan as "exempt personnel." Exemption from certain taxes, customs restrictions and immigration regulations. Government furnished SOFA-status ID pass/badge and car passfor the incumbent. Purchasing privileges that authorize purchases at the Defense Commissary Agency (DECA) facilities Use of Army and Air Force Exchange Service (AAFES) facilities and concessions. This includes purchase of petroleum and oil products. Use of Morale, Welfare Recreational facilities. This includes "Officer's Open Mess" (club) membership, MCB Butler, as determined by each respective club, including access to all recreational activities (gyms, golf course, pools, etc.). Military Postal Service support on a space available basis. Use of Military Banking Facilities. Contractor use of Government vehicles from a scheduled pool of vehicles, for official use during working hours, limited to meeting the requirements of use and the availability of vehicles. The contractor is responsible for providing transportation to and from work. Contractor ability to obtain a privately owned vehicle (POV) operator's license, termed œSOFA license  and registration of a POV.
