- (i.)This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Additionally, this announcement constitutes the only solicitation for The Midwest Health Care Network (VISN 23); The Midwest Health Care Network is seeking proposals from interested vendors to award multiple Basic Ordering Agreements (BOA) for each community identified in Table a - Locations & Point of Contact Information for Interested Parties. Interested Community Nursing Homes should review paragraph (v.) of this combined synopsis / solicitation for instructions on how to initiate a request to participate in the Midwest Health Care Network Community Nursing Home (CNH) program. (ii.)The reference / solicitation number is VA-263-12-R-0022 and the solicitation is issued as a Request for Proposals (RFP). The combined synopsis / solicitations are being issued as an unrestricted Request for Proposals (RFP) by the Midwest Health Care Network (VISN 23) under statutory Authority 41 U.S.C. §253 (c) (5) and 38 U.S.C. §1720. The nursing home facility shall ensure that care meets the health needs and promotes the maximum well being of VA patients. Nursing home care will be furnished to ensure the total medical, nursing, and psychosocial needs of VA beneficiaries. Physician visits, laboratory, x-ray, and other special services for VA patients will be at the same frequency as that provided to other patients at the nursing home receiving the same or comparable level of care. In addition, the care provided will include room, meals, nursing care, and other services or supplies commensurate with the VA authorized level of care, without extra charge. The per diem rate(s) will include the cost of medical care, oral medications, laboratory, x-ray, and other special services authorized by VA, unless otherwise specifically accepted. Facility must have and maintain required applicable state licensing and minimum insurance requirements for Nursing Home Facilities. (iii.)The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication through Federal Acquisition Circular 2005-53. Provisions and clauses incorporated by reference have the same force and effects as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv.)North American Industry Classification System (NAICS) Code is 623110 with the small business size standard of $13.5 million (USD) applies to this solicitation. (v.)Procedure for initiating a Basic Ordering Agreement (BOA) for any of the specified communities indentified in Table a - Locations & Point of Contact Information for Interested Parties are as follows; interested Community Nursing Homes need to complete Standard Form (SF) 129 - Solicitation Mailing List Application; VA Form 10-1170 - Contract Award for Furnish Nursing Home Services to Beneficiaries of the Veterans Administration, and a descriptive cover letter to the applying nursing home. Both forms have been attached to this combined synopsis / solicitation for your convenience. Once the Community Nursing Home completes the required forms listed above, mail the completed forms to the respective Midwest Health Care Network Contract Officer identified in Table a - Locations and Point of Contact Information for Interested Parties. Additionally, interested Community Nursing Home (CNH) have the option to fax or e-mail a digital copy of the completed initiating forms to the respective Midwest Health Care Network Contract Officer. Submission of electronic proposal through e-mail, offerors are responsible to ensure the respective Midwest Health Care Network Contract Officer received a copy of the proposal (initiating packet). (vi.)This requirement is for Community Nursing Homes to provide Long-Term Care (LTC) services through Basic Ordering Agreements (BOA) to correspond to eligible veteran's geographic preferences and institutional Long-Term Care (LTC) needs. (vii.)The Midwest Health Care Network anticipates awarding multiple Basic Ordering Agreements (BOA) (at least one per indentified site) to responsive / responsible offeror(s) whose offeror conforming to the solicitation will be most advantageous to the Government per Federal Acquisition Regulation (FAR) Part 13.106-2 for a period not to exceed 12-months, with four (4) one-year option periods, at the discretion of the Government within prescribed time periods on or before December 31, 2011. The Government reserves the right not to award a Basic Ordering Agreement (BOA), if it determines that it is not in the "best interest" of the Government. (viii.)The provision at Federal Acquisition Regulation (FAR) 52.212-1 Instructions to Offerors - Commercial Items (June 2008), applies to this solicitation and incorporate the following addenda to 52.212-1; 52.204-6 Data Universal Numbering System (DUNS) Number; 52.212-4 Contract Terms and Conditions - Commercial Items (June 2010). (ix.)The provision at Federal Acquisition Regulation (FAR) 52.212-2 Evaluation - Commercial Items (Jan 1999) applies to this requirement. The Government will award a firm fixed price Basic Order Agreement (BOA) resulting from this solicitation to the responsible offeror(s) whose offer(s) conforming to the solicitation will be most advantageous to the Government. The Midwest Health Care Network will publish an electronically consolidated notification via a Federal Business Opportunities Award Notice once all sites Basic Ordering Agreement (BOA) have been awarded; the award posting notice on the Federal Business Opportunity (FBO) Website (http://www.fbo.gov/), will serve as non-awardees official electronic notification of award. (x.)The provision at Federal Acquisition Regulation (FAR) 52.212-3 Offeror Representations and Certifications-Commercial Items (May 2011) applies to this requirement. The contractor shall either: 1. Return a completed copy of this provision with their quote (a copy of the provision may be attained from (http://www.arnet.gov/far); or 2. (xv.)OFFERORS RESPONSES (CONSISTING OF SF 129, VA FORM 10-1170, AND A DESCRIPTIVE COVER LETTER) ARE DUE FRIDAY, NOVEMBER 18, 2011, by 4:00 PM (Central Time) TO THE RESPONSIBLE CONTRACTING OFFICER.
