Solicitation Number: VA-241-12-Q-0098 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-12-Q-0098 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-53, July 05, 2011. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 541990, and the size standard is $7.0 million. (v) This requirement consists of 1 line item 1.Three day conference for veteran's with traumatic brain injury, autism and cognitive disabilities. Contractor shall submit a total price quote per individual attending the conference. (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: 1.Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified: This conference, shall address related health, supported education and employment challenges faced by returning Veterans from Iraq, Afghanistan and related conflicts. Additionally, we will seek to identify national capacity-building programs, healthcare, economic development and reintegration models which could aid both our returning Vets and our US-Japanese Exchange colleagues. Our goal for this Symposium is to share existing "Best Practice" models and to identify innovative solutions to benefit: national reconstruction, healthcare and economic capacity. These programs can then grow to aid large returning Veteran populations with PTSD and TBI, as well as offering models for our Japanese Exchange partners whose population, healthcare systems and economic opportunities were greatly impacted by the 2011 Tohoku Earthquake and Tsunami. In addition, this program aims to instill a vision of program development and operations based on state-of-the-art best practices of vocational and psychosocial rehabilitation (PSR) for Veterans diagnosed with posttraumatic stress disorder (PTSD), traumatic brain injury (TBI), spinal cord injury (SCI), or Polytrauma blast injuries. Participants shall receive updates on the clinical treatment techniques and research findings of best practices, the Department of Veterans Affairs, Compensated Work Therapy's Supported Employment (SE) vocational services and Supported Education. The program will engage PTSD and CWT clinical treatment providers in the concepts of integrating care with vocational rehabilitation specialists and in addressing workplace issues and performance in the context of treatment, counseling, and recovery efforts. Specific Requirements: 1.Training Workshop- The contractor shall demonstrate specific expertise relating to Polytrauma, PTSD, TBI, SCI, and the US. Japanese Exchange program as it relates to veterans. Contractor must demonstrate successful past performance for previous conferences conducted. The contractors shall provide a detailed work plan with all material and agenda that will be delivered. Contractor shall submit the dates, times, places, and material from three conferences that were conducted relating to the required subject matter. All conference presenters must be experienced PTSD and CWT clinicians.. (vii) Delivery FOB Destination to VA Medical Center: Site for work conference will be determined upon award. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)Technical capability of the item offered to meet the Government requirement (ii)Price (iii)Past performance and experience with Polytrauma, PTSD, TBI, SCI, and the US. Japanese Exchange program as it relates to veterans. All evaluation factors other than cost or price, when combined, are approximately equal to cost or price. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification. A copy of the provision may be attained from http://www.arnet.gov/far; (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: apply to this solicitation with the following addenda; VAAR 852.273-76 Electronic Invoice Submission, and VAAR 852.246-71 Inspection.
