Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number, VA260-12-Q-0102, is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective August 2, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 492110, Size Standard: 1500 employees. This procurement is unrestricted. Statement of Work- Walla Walla, WA, 99362 VA Center, Courier Requirement Contact Information 1.Project Scope: Provide the necessary courier services to transport Laboratory Specimens, contaminated and clean supplies, linens and Pharmaceuticals to and from Community Based Outpatient Clinics (CBOC) in the following sites to Jonathan M. Wainwright Memorial Veterans Affairs Medical Center Pathology and Laboratory Service and Warehouse, Walla Walla WA. 2.The proposal must include cost for pick up and delivery round trip including dry ice, Laboratory specimens, contaminated and clean instruments, Pharmaceuticals (non-narcotic), mail in secured( locked bags), linens and clean supplies. 3.The contractor will be responsible for the following: a. Pick up of Laboratory Specimens, contaminated and clean instruments, Pharmaceuticals, mail and clean supplies from i.Yakima VA clinic (717 Fruitvale Blvd Yakima WA) ii. Richland VA clinic (625 Jadwin Avenue Suite 250 Richland WA) iii. Lewiston VA CBOC (1630 23rd Ave Suite 301 and 401 Lewiston Idaho 83501) iv. La Grande VA CBOC (202 12th St., La Grande Oregon 97850) v.Grangeville VA CBOC( 711 north Street, Grangeville, ID 83530) vi.Enterprise VA Tele-Health Clinic (401 NE 1st, Enterprise, Oregon 97828 vii.Morrow VA Tele-Health Clinic (Port of Morrow,2 Marine Street, Boardman, OR 97818 Returning to Jonathan M. Wainwright Memorial Veterans Affairs Medical Center (Walla Walla VA) Laboratory (Building 86 room 124) and Warehouse (Building 81) and returning supplies to the respective Clinics the following or prior to specimen pick up. b.Specimen pick up schedule is daily for all sites excluding Federal Holidays and on cancellation of Clinics with advanced notice from either the Walla Walla Core Lab or the Clinic managers. ? FEDERAL HOLIDAYS The Federal Government observes the following days as holidays. If the date falls on a Sunday, the Government holiday is the following Monday. In addition to the days designated as holidays, the Government observes the following days: "Any other day designated by Federal Statute "Any other day designated by Executive Order "Any other day designated by the President's Proclamation c.Pick up times would be no earlier than 1pm at Yakima, 2:30 pm Richland clinics; Lewiston pickup would be no earlier than 2 pm. La Grande pickup would be no earlier than 2 pm. Enterprise pick up would be no earlier than 1:30 pm, Boardman pickup no earlier than 1 pm and Grangeville pick up would be no earlier than 12 pm. d.Delivery time to the Walla Walla VA Laboratory would be no later than 1630. e.Pick up of supplies for the return trip would be not later than 1620 from the Warehouse Bld 81 Walla Walla VA. f.The courier service will provide notification to the Walla Walla Laboratory manager or designee via telephone if there is to be a delay beyond the set delivery or pick up times. g.Specimens must be transported in conditions required to preserve specimen integrity. Accommodations must be made to transport Room Temperature, Refrigerated Temperature and Frozen specimens. Specimens must arrive in optimal conditions to ensure specimen viability and integrity. h.Frozen Specimens will be transported on Dry ice VA Walla Walla will provide dry ice for the courier or purchase price of 1 lb each day may be added to the contract. i.VA Walla Walla will provide a courier cooler/transport for the driver to use in transporting the specimens. It has separate compartments for Frozen, Refrigerated and Room Temperature specimens. The containers are engineered for safe storage within a "room temperature" condition for a short period of time j.All vehicles shall have heating and air conditioning to maintain a temperature between 65 and 85 degrees Fahrenheit. Vehicles shall not be left in extreme heat or cold while specimens, pharmaceuticals, and films are being transported. k.Specimens and other contaminated materials must be transported in separate areas of the transport vehicle from clean supplies to prevent contamination according to IATA regulations. l.Specimen and Item tracking will be provide via shipping manifest generated by VA Walla Walla and the Clinics to the locations for delivery. The manifest is signed by the driver and presented to the final destination of the run of the day. m.Courier service must provide written and/or electronic documentation of annual training of courier personnel in current IATA hazardous transport regulations to the Laboratory Manager at least yearly or on request. n.The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and Information system security as delineated in this contract.839.201 Contract clause for Information and Information Technology Security: The Contractor shall ensure the confidentiality of all patient information being transported and will be held liable in the event of breach of confidentiality. The contractor shall comply with the provisions of the Federal Privacy Act of 1974 (Public Law 93-579), the comprehensive Alcohol Abuse and Alcoholism Prevention, Treatment and Rehabilitation Act of 1972 (Public Law 93-282), and the Drug Abuse Office and Treatment Act of 1972 (Public Law 93-255), and any other statutes regarding confidentiality of patient information. o.Contractor shall wear identification badge. p.The Contractor shall comply with all applicable laws, ordinances and regulations (Federal, state, city or otherwise) covering its operations under the contract q.Quality review will be ongoing (reviewed at least quarterly) and include late pick up or delivery time. Specimens arriving not within temperature specifications. Lost or damaged items. No more than 1 occurrence per month unless mitigating circumstances (road closure or extreme weather) will be allowed. Failure to provide the service (no pick up) will have no occurrences unless mitigating circumstances. r.Feedback will be provided to the service on quality review. s.Return Supplies must be held in secure temperature controlled storage until delivered to the CBOC. t.The contract to continue for 5 years with yearly renewal options and may be terminated with 30 day notice from either party. u.Cost to be + Fuel Surcharge (West Coast index on www.epa.gov ). v.Billing per contract to be monthly basis billed on the 15 of each month. w.Pathology and Laboratory Manager will act as Contracting Officer Technical Representative (COTR) for this contract. x.Contract to commence as soon as possible after December 19, 2011.
