Description

The Indian Health Service White Earth Health Center in Ogema, MN intends to contract using other than full and open competition for the following services: Telephone Services Features: - Voicemail - Data service - Telephone service Hardware: - Phone service for Naytahwaush Health Center, MN Vendor: Qwest Government Services Inc DBA: Centurylink QGS 10300 Eaton PL Ste 440 Fairfax, VA 22030 DUNS: 178617031 Centurylink is the only responsible vendor capable of serving the IHS WEHC. Award to any other source would create substantial duplication of cost to the Government that is not expected to be recovered through competition. Period of Performance: Base year: 11/14/2011 - 11/15/2012, $4,155.46 / Approximate Annual Cost Opt Yr 1: 11/14/2012 - 11/15/2013, $4,155.46 / Approximate Annual Cost Opt Yr 2: 11/14/2013 - 11/15/2014, $4,155.46 / Approximate Annual Cost Opt Yr 3: 11/14/2014 - 11/15/2015, $4,155.46 / Approximate Annual Cost Opt Yr 4: 11/14/2015 - 11/15/2016, $4,155.46 / Approximate Annual Cost Prospective vendors may submit a notice of interest and capabilities in response to this notice. This notice of intent is not a request for competitive proposals. All responsible sources may submit a proposal which shall be considered by the Government. Information received shall be considered solely for the purpose of determining whether or not to conduct a competitive procurement. Determination by the Government not to compete the proposed contract action upon receipt of any responses shall be within the sole discretion of the Government.
