Description
This is a notice of intent to issue a solicitation for a firm fixed price commercial contract for The personal services of a part time Occupational Therapist at Winn Army Community Hospital, Fort Stewart, GA. A brief description of qualifications and duties are detalied below, but are not limited to the duties listed. Additional information will be detailed in the Performance Work Statement. The Occupational Therapist will also be required to perform services at other locations on Fort Stewart as well as travel (TDY) for training in accordance with the Joint Travel Regulation. This requirement will be advertised 100% Small Business. Qualifications: Have at least (2) two years of experience within the last five years in pediatric occupational therapy for ages 0-to 36 months with special needs. Must be Board Certified and currently licensed in Occupational Therapy in a least one state or territory in the United States. Must have and maintain a Health Care Provider Basic Life Support Certification. A valid driver's license is required. all certified personnel shall possess and maintain certifications as required for their area of discipline. All personnel shall have satisfactorily completed residency training in an accredited Occupational therapy program approved the American Occupational Therapy Association(AOTA) an American (Occupational Therapy) residency program and been awarded a diploma for satisfactory completion of said residency. All personnel shall have current medical board certification in (AOTA). Knowledge of the principles and practices of family-centered care as it relates to EDIS described in the Individuals with Disabilities Act (PL 105-17, Part C), and 32 CFR 80 to participate appropriately in the development and implementation of IFSP. Skill necessary to independently evaluate and treat pediatric patients, especially from 0 - 3 years of age, with a wide range of diagnoses, delays and risks from the minimally involved child to the severe and profoundly developmentally disabled child. Duties: Complete Developmental Screenings, Intake Process and accurately explain the concepts of EI to Families. Evaluates and facilitates functional performance of children who may be eligible to receive services that are suspected of sensory-motor dysfunction. Evaluate children and determine current levels of functioning. Counsels family members in regards to evaluation results and establishes long/short-term objectives for the intervention plan. Provides teaching and training to family, extended family members, and child-care providers to implement a home-based intervention program. Reassess the functional performance as necessary. Prepares written and oral response of intervention services as needed. This requirement will be for a base period and four (4) option years. Listed as follows: Base period: 15 February 2012 - 30 September 2012 Option 1: 1 October 2012- 30 September 2013 Option 2: 1 October 2013 -30 September 2014 Option 3: 1 October 2014 -30 September 2015 Option 4: 1 October 2015 -30 September 2016 The solicitation will be issued on or about December 5. 2011 through the FedBizops website at www.fbo.gov with a closing date on or about December 20, 2011.
