Description
SOURCES SOUGHT ONLY ( This is not a formal Request for a Proposal ) 1. SCOPE 1.1 Background The Pantex Plant is a Department of Energy (DOE)/National Nuclear Security Administration (NNSA) owned, contractor-operated facility, managed by B&W Pantex, LLC. Pantex was constructed by the U.S. Army for production of conventional ordinance during World War II. Today the Plant's mission is to assemble nuclear weapons for the nation's stockpile, disassemble nuclear weapons being retired from the stockpile, evaluate repair retrofit nuclear weapons in the stockpile, sanitize components from dismantled nuclear weapons, provide interim storage for plutonium pits from dismantled nuclear weapons, and develop, fabricate, and test chemical explosives and explosive components for nuclear weapons to support DOE initiatives. The Pantex Plant is located in Carson County, approximately 17 miles northeast of Amarillo, Texas. 1.2 Objective The objective of this statement of work is to obtain a provider who is able to offer an accidental death and dismemberment (AD&D) policy to a group of highly trained first responders. 2. Applicable Documents No applicable documents 3. Requirements 3.1 General 3.1.1 The contractor shall be able to offer a AD&D policy limited to a maximum amount of $1,000,000 per insurer, per sanctioned activity. 3.1.2 The policy should only cover the duration of the sanctioned activity as specified by Pantex. (Short-term, as needed policy) Duration is defined as from the onset of deployment to include travel to the sanctioned activity, thru the return of the insured to include travel. 3.1.3 The policy will cover those designated as members of the First Response team, who have received specialized training with weapons of mass destruction, as specified by Pantex. 3.1.4 The contractor will provide coverage to include death or injury to an insured person as a result from a deliberate act by another person or persons while on sanction activity of the policyholder. 3.1.5 The contractor will provide coverage to include death or injury to an insured person as a result of injury or suicide when committed in the face of immediate threat of death or torture by another person, in order to end insupportable suffering caused by a covered occurrence. 3.1.6 The contractor will allow Pantex to make notification to the contractor when a policy is in need via a written form. Pantex will provide the contractor with the dates of the sanctioned activity and the number of first responders to be covered. Contractor will write policy according to the information specified, possibly retro-effective dating coverage if advance notice of an event is not possible. 3.1.7 The contractor will invoice on a per person per trip amount after each sanctioned activity. 3.2.1 Security Requirements 3.2.2 Assigned contractor personnel must comply with all protection controls that relate to their work as communicated by the Subcontract Technical Representative (STR). 4. Government-Furnished Property There will be no government furnished property provided during this project. 5. Government-Furnished Facilities There will be no government furnished facilities provided during this project. 6. Deliverable Data Data will be made available either electronically or by paper. 6.1.1 Mutually agreed upon form to make application for policy. 6.1.2 Invoice of total charges after each sanctioned activity. 7. Schedule 7.1.1 Vendor should be prepared to offer policy by January 1, 2012.
