Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number W9127Q-12-T-0001 is issued as a Request for Quotation. The solicitation and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-54. This solicitation is 100% Set-Aside for Small Business. NAICS for determination of size status is 532490. CLIN 0001: LEASE OF CONTAINER HANDLER REACH TRUCK, 1EA. The Contractor shall provide equipment and maintenance services for the leased equipment. All work shall be accomplished in accordance with the best commercial practices for forklift services. 35k Forklift, not to exceed 3,500 pounds, shall able to move large containers, objects, and equipment. Forklift shall have an 18' reach and able to stack 3 containers high. Contractor will provide equipment that is fully functional with no deficiencies. The Forklift Reach Stacker with a fixed cab, single seat, front and top wipers, 3 speed power shifts with auto shift transmission, 2 headlights, 2 tail lights, 2 front marker lights, side shift and fork positioned forks, heater, defroster, fan and or air conditioner. Rent will be paid monthly in arrears, upon receipt of a proper invoice from the Contractor. Proposal shall include (broken down into each item) monthly rental, all freight, warranty dealer set-up fees and installation at Camp Shelby, Mississippi. Base period will be from actual date of delivery through the following 12 months, with four option years. CLIN 0002: OPTION YEAR ONE (12 months). CLIN 0003: OPTION YEAR TWO (12 months). CLIN 0004: OPTION YEAR THREE (12 months). OPTION YEAR FOUR (12 months). Equipment shall be delivered to Camp Shelby, Mississippi. Provide Specifications with your quote. The following FAR provisions apply: FAR 52.212-1, Instructions to Offerors - Commercial Items. Offerors must include a completed copy of FAR 52.212-3, Offeror Representations and Certifications - Commercial Items and DFARS 252.212-7000, Offeror Representations and Certifications - Commercial Items, with its offer. If offeror is registered in ORCA, Reps and Certs are not required; however, offeror must indicate such with their proposal. FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Order-Commercial Items. Applicable FAR Clauses cited within 52.212-5 are as follows: 52.204-10, Reporting Executive Compensation and First-Tier Subcontract Awards; 52.219-6, Notice of Small Business Set-Aside; 52.219-28, Post-Award Small Business Program Representation; 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; FAR 52.222-50, Combating Trafficking in Persons; 52.225-13, Restrictions on Certain Foreign Purchases; 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration; 52.233-3, Protest After Award; 52.233-4, Applicable Law for Breach of Contract Claim; 52.222-3, Convict Labor; 52.222-19, Child Labor-Cooperation with Authorities and Remedies; 52.222-36, Affirmative Action for Workers with Disabilities.
