Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The Request for Quote number is RFQ VA-249-12-R-0083. Quotes are due no later than 4 pm EST on 23 November 2011, and shall be sent via email to Patrice.arvie@va.gov. Fax quotations will also be accepted. Fax number is (615)225-5431. Email is preferred. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a Quote for the provision of Mobile Lithotripsy equipment and related personnel and support of the Surgical Services, at the James H. Quillen VA Medical Center (JHQVAMC), Mountain Home, Tennessee. This is a solicitation for number VA-249-12-R-0083 is issued as a Request for Quote (RFQ) and constitutes the only solicitation and a written solicitation will not be issued. This acquisition will be processed under Commercial Item Procedures with an approximate delivery period of 01 January 2012 through 30 September 2012 and three one year option periods. This procurement is solicited on an unrestricted basis. Quotes will be evaluated in accordance with the requirements of Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items in conjunction with the procedures of FAR Part 15.101-2. When all Quote evaluations are complete, the Contracting Officer will make judgments required, all factors considered, in selection of the offeror whom he/she considers having the ability to perform overall the contract in a manner most advantageous to the Government. The Government may award a contract on the basis of initial offers received without discussions. Therefore each initial offer should contain the offer's best terms from a cost/price and technical standpoint. Award will be made to the lowest price technically acceptable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43, subject to the availability of funds, and solicited on an unrestricted basis for NAICS 621999, with a size standard of $10.0. All responsible sources may submit a Quote which shall be considered by the agency. See attached Statement of Work SCHEDULE OF SERVICES AND COSTS The Contractor shall provide the James H. Quillen VA Medical Center (JHQVAMC), Mountain Home, TN, with mobile Lithotripsy services. Services shall be provided at the James H. Quillen VA Medical Center, Mountain Home, TN 37684. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS NUMBER and completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items. The following evaluation factors apply to this solicitation. Contractor shall provide responses to all evaluation factors. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. TECHNICAL EVALUATION Transmittal Instructions: The technical Quote shall be submitted as separate from the price Quote. NO COST INFORMATION SHALL BE INCLUDED IN THE TECHNICAL QUOTE. The price Quote shall be submitted as a second enclosure and shall contain all information relative to cost and pricing. The offeror will provide three (3) copies of the Technical Quote and one (1) copy of the Price Quote if you are mailing Quotes. The offeror can also e-mail the Technical Quote and the Price Quote as separate attachments to the same e-mail to Patrice.arvie@va.gov. TECHNICAL QUOTE The Offeror shall demonstrate past performance by identifying a minimum of three (3) references with similar size and patient complexity located in the area of the JHQVAMC Medical Center, Mountain Home, Tennessee. References shall at a minimum include Name of Organization, Point of Contact, and Point of Contact telephone number. Offeror shall propose how they will meet the minimum general requirement for this Quote. TECHNICAL EVALUATION CRITERIA 1. Accreditation, if applicable. The contractor will be required to demonstrate compliance with JOINT COMMISSION standards and/or other medical credentialing/oversight bodies by providing evidence of the following: JOINT COMMISSION certification, the ability to meet all JOINT COMMISSION standards/certification or the ability to meet all credentialing/oversight bodies' standards/guidelines. 2. Organization and Management Written material provided to the JHQVAMC Medical Center, Mountain Home, Tennessee will include the following placed in binders and tabbed according to an index: or sent as separate attachments to one e-mail if you are submitting electronic Quotes. 3.Copy of Certificates and any other certification or license required to conduct business per this contract (if applicable); Current Copy of competency related to lithotripsy equipment: Turnover rate of support and professional staff; Current company background check; Current drug test certification PPD test results; Proof of rubella testing; PAST PERFORMANCE: List at least three (3) references that can attest to your or your group's ability to perform Lithotripsy Procedures. Please include name, address, telephone number and point of contact. These three references should not be from within your group practice. QUALITY ASSURANCE: Provide one (1) copy of your Quality Assurance Plan. PRICE: To be evaluated by the Contracting Officer and designees. EVALUATION The Government will award a contract resulting from this solicitation to the offerors whose offer conforms to the solicitation and is most advantageous to the Government, price and other factors considered. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
