- THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A SOURCES SOUGHT SYNOPSIS ONLY. THE PURPOSE OF THIS SOURCES SOUGHT SYNOPSIS IS TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED SOURCES AND THEIR SIZE CLASSIFICATIONS (HUBZONE 8(a), 8(a), HUBZONE, SMALL, SMALL DISADVANTAGE AND LARGE BUSINESS) RELATIVE TO NAICS CODE 621511 (SIZE STANDARD OF $13.5 MILLION AVERAGE ANNUAL RECEIPTS FOR PRECEDING THREE FISCAL YEARS). RESPONSES TO THIS SOURCES SOUGHT SYNOPSIS WILL BE USED BY THE GOVERNMENT TO MAKE APPROPRIATE ACQUISITION DECISIONS, SEE SUBMITTAL REQUIREMENTS BELOW. AFTER REVIEW OF THE RESPONSES TO THIS SOURCES SOUGHT ANNOUNCEMENT, A SOLICITATION ANNOUNCEMENT WILL BE PUBLISHED IN FEDBIZOPPS. RESPONSES TO THIS SOURCES SOUGHT ANNOUNCEMENT ARE NOT ADEQUATE RESPONSE TO THE SOLICITATION ANNOUNCEMENT. ALL INTERESTED OFFERORS WILL HAVE TO RESPOND TO THE SOLICITATION ANNOUNCEMENT IN ADDITION TO RESPONDING TO THIS SOURCES SOUGHT ANNOUNCEMENT. Travis AFB is seeking sources to perform urinalysis testing for David Grant Medical Center, Travis AFB, CA. The Contractor shall provide all necessary labor, materials, and transportation to provide this service. Period of Performance will be one base year with four one-year option periods. Responses to this notice should include company name, address, point of contact, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) and must respond to the following questions: Questions: 1) Is your business a large or small business? 2) If small, does your firm qualify as a small, emergency business, or a small, disadvantaged business? 3) If disadvantaged, specify under which disadvantaged group and is your firm certified under Section 8(a) of the Small Business Act? 4) Is your firm a certified "hub zone" firm? 5) Is your firm a woman-owned or operated business? 6) Is your firm a certified Service-Disabled Veteran Owned and Veteran-Owned? 7) Does your firm have an Environment Management System Certification (EMS)? 8) What qualifications/certifications does your firm have? 9) What is the average work experience of your medical support personnel? 10) Does your firm perform background checks? 11) Does your firm have a quality assurance plan in place? What quality assurance methods and quality standards does your firm use? 12) Do you currently have a government contract? Interested businesses are required to submit a Statement of Capabilities (SOC) demonstrating: 1) Ability to manage the services, description of past performance on previous contracts, dollar value of each contract, number of employees for each, the types and range of skills of employees for each, and point of contact and telephone number of the contracting activity for each contract. 2) A capability statement, including summary of relevant performance history (with points of contact and applicable telephone numbers, FAX numbers, addresses and email addresses, if available) within the past three years. This is not a request for proposals and in no way obligate the Government to award any contract. Responses must be received in writing no later than 27 Nov 2011. Firms responding to this announcement which fail to provide all of the required information cannot be used to help the Government make the acquisition decisions which is the intent of this synopsis. Industry Specific Questions: 1. Is your Urine Chemistry Analyzer and Urine Microscopy Analyzer designed under one platform being that they are made by the same manufacturer and are meant to function as one unit? 2. If your Urine Chemistry Analyzer and Urine Microscopy Analyzer operate as separate units, what are the model numbers and manufacturers for each unit? 3. What other medical centers do you currently provide this equipment to? 4. Is your Automated Urinalysis Platform equipment FDA approved and capable of performing automated analysis on all body fluid counts to include CSF, Pleural, Peritoneal, Pericardial, Synovial, Peritoneal Lavage, and Peritoneal Dialysate? 5. Does the total footprint of the unit exceed the requirements 24H x 55W x 36D (61Hx 140W x 91D cm) excluding workstation? 6. Can your equipment perform digital imagery microscopic review and provide image storage capabilities? 7. Does your Urine Chemistry Analyzer and Urine Microscopy Analyzer have onboard storage capability of 10,000 or more results? 8. Can your Urine Microscopy Analyzer and Urine Chemistry Analyzer analyze sample volumes of 2.0ml or less? 9. Does your Urine Microscopy Analyzer and Urine Chemistry Analyzer have a walk-away capacity of at least 60 samples? 10. Does your Urine Microscopy Analyzer and Urine Chemistry Analyzer system utilize keyboard entry, and barcode, as a minimum, to enter sample identification? 11. Do your units possess Code 39 Barcode capability? 12. Is your Platform Bi-Directional and is there an interface with the Composite Healthcare System (CHCS) currently operational at a DoD site? Where? 13. Is your equipment capable of performing Flow Cell (Cytometry) with Digital Imaging and Reflectance Photometry? 14. Is your Urine Microscopy Analyzer able to identify the following particles: RBC, RBC Clumps, WBC, WBC Clumps, Squamous Epithelials, Non-Squamos Epithelials (Renal Epithelials, Transitional Epithelials), Hyaline Casts, Unclassified Casts (Granular, Cellular, Waxy, Broad, RBC, WBC, Epithelial, Fatty), Crystals (Calcium oxalate, Triple phosphate, Calcium phosphate, Uric acid, Calcium carbonate, Leucine, Cystine, Tyrosine, Amorphous), Yeast (Budding, hyphae/psuedohyphae), bacteria, Spem, Mucus, Fat, Oval Fat Bodies, Trichomonas ? 15. Is your Urine Chemistry Analyzer able to perform the following chemistry tests: Bilirubin, Urobilinogen, Ketones, Glucose, Protein, Blood, pH, Nitrite, Leukocytes (Leukocyte Esterase), Ascorbic Acid, Specific Gravity, Color, Clarity? This notice is to help Travis AFB in determining potential sources only. No solicitation is currently available. All firms responding to this sources sought notice are advised that their response to this notice is not a request to be considered for a contract award. If a solicitation is issued, it will be posted later at https://www.fbo.gov and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. A response to this sources sought synopsis is NOT a request to be added to a prospective bidders list or to receive a copy of the solicitation.
