- hippa form child protection form sow sf 33 form The Albuquerque Area Indian Health Service has a requirement for Diagnostic Ultrasound Techologist Services for the Albuquerque Indian Health Center, in Albuquerque, New Mexico. The estimated requirements are for an estimated 2080 hours per contract year. The contractor will be required to perform up 36-40 hours per week, depending on the clinical need and within a eight-hour day. The contract will be awarded as a IDIQ contract for base year and three one year options for renewal. The contractor must have a certificate Diagnostic Medical Sonographer with demonstrated experience in performing basic route x-ray as required. All interested offerors must meet the evaluation factors. Refer to Request for Proposal 12-242-SOL-00005 when requesting the solicitation package.
