- MODIFICATION TO PRESOLICITATION NOTICE: The purpose of this modification to the Presolicitation Notice is provide the responses to the questions that have been posted and received regarding the Radiation Oncology Solicitation RFP VA-250-11-RP-0016. The following questions and responses are being provided below: QUESTIONS AND RESPONSES FOR LONG-TERM RADIATION ONCOLOGY/RADIATION THERAPY SERVICES SOLICITATION SOLICITATION NUMBER: VA-250-11-RP-0016 1. My first question is; In filling out form 1449, I must complete blocks 12, 17, 23, 24 & 30. 17a. what is my Contractor/Offeror Code? Facility Code? Where do I acquire these? Response: Blocks 12, 17, 23, 24 & 30 must be completed. On Block 17a, the Code and Facility Code only need to be completed for the Contractor/Offeror if they have them. 2. In Block 23 is this part of the fee schedule? Is this also the same as Block 11? Response: While Block 23 is part of the unit price on the SF 1449, it must be completed on the Price Schedule, which is located on pages 28 through 29 of solicitation. Block 11 refers to the Price Schedule, which is located on pages 28 through 29 of the solicitation. 3. Full submission means to include the two copies of form 1449 signed and delivered to the issuing office? Response: Full submission means to include the two copies of the entire solicitation and the Offeror ™s proposal that will need to be submitted and provided to the issuing office by the Offer Due Date, Tuesday, November 29, 2011, 12:00 Noon Eastern Standard Time (EST). 4. The scheduled fee is being requested in an excel format? Can this be submitted electronically via the web site or email? Response: The schedule fee that is being requested in an excel format can be submitted electronically via the web site or email. However, the schedule fee is due on the same Offer due date as the solicitation, Tuesday, November 29, 2011, 12:00 Noon Eastern Standard Time (EST). 5. Registrations “ How many and which ones? I know to register with the CCR and pay the fee. Have NPI number register with NIH CPS, are there others? Response: All Contractors that want to do business with the Federal Government must be registered in Central Contractor Registration (CCR) (http://www.ccr.gov/). All Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) that are providing their proposals for this solicitation must be registered in VetBiz (http://www.vip.vetbiz.gov/general_user/register/default.asp). All Contractors must be registered with Dun and Bradstreet. All Offerors responding to this solicitation must provide their Data Universal Numbering System (DUNS) Number and Tax Identification Number, which is referenced on page 6 of the solicitation. Also, All Offerors must complete and return all information requested in this solicitation, including the information designated in Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offerors ”Commercial Items located on pages 78 through 82 of the solicitation. 6. Once paperwork completed what is the next step or when will a decision be made? Response: Once paperwork has been completed and all offers/proposals have been received by the Offer Due Date, Tuesday, November 29, 2011, 12:00 Noon Eastern Standard Time (EST), all of the proposal will be evaluated by the Source Selection Board. A decision will be made when all of the evaluations have been completed and a successful Offeror has been determined for this contract. All interested parties will be provided notification when a decision has been made. 7. For the CSCA “ Contractor Security Control Assessment (pg 66) “ Who signs Part 3b.? The COTR or my staff? Response: Since the Vendor ™s information is included in Part 3a, the Veterans Affairs Ambulatory Care Center (VAACC) Contracting Officer ™s Technical Representative (COTR) signs the first block of Part 3b., the Vendor ™s Executive Officer ™s name goes in the 2nd block of Part 3b. and the VAACC ™s Information Security Officer (ISO) signs in the 3rd block of Part 3b. 8. I know I asked the questions before but do I return all 105 pages? Response: The entire solicitation ”all 105 pages ”with offer/proposal must be submitted and returned by the Offer Due Date, Tuesday, November 29, 2011, 12:00 Noon Eastern Standard Time (EST). 9. Acknowledgement of Amendments pg 6. Where are the amendments to acknowledge too? Are these it? FAR 52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (JAN 2011) FAR 52.209-8 UPDATES OF INFORMATION REGARDING RESPONSIBILITY MATTERS (APR 2010) 52.224-1 PRIVACY ACT NOTIFICATION APR 1984 52.224-2 PRIVACY ACT APR 1984 FAR 52.252-1 SOLICITATION PROVISIONS INCORPORATED BY REFERENCE (FEB 1998) FAR 52.252-2 CLAUSES INCORPORATED BY REFERENCE (FEB 1998) RESPONSE: There are no amendments to acknowledge for this solicitation. The FAR clauses that have been referenced above are not the amendments to acknowledge. The Offer Due Date for RFP VA-250-11-RP-0016 for the procurement of Radiation Oncology/Radiation Therapy Consultation and Treatment Services for the VAACC, Columbus, OH, is November 29, 2011 at 12:00 Noon Eastern Standard Time (EST).
