Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number, VA260-12-Q-0151, is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective August 2, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 811219, Size Standard: 16.5 Million. This procurement is unrestricted. STATEMENT OF WORK FOR Service Agreement for BIOMEK 2000 21 November 2011 1.Contracting Officers POC: Heather Trevisanut 2. 3.Contract Title: Service Agreement for BIOMEK 2000 4.Background: The VA in requires an annual service agreement for the following equipment: BIOMEK 2000 Serial Number 4324409 BIOMEK 2000 P200L Single Tip ToolSerial Number 4844390 BIOMEK 2000 P100 Single Tip ToolSerial Number 4386317 BIOMEK 2000 Wash Tool ( 8 Channel)Serial Number 4394235 BIOMEK 2000 MP200 8 - TipSerial Number 4390466 The service agreement covers both planned and corrective maintenance. 5.Term of the Contract: The performance period is from contract award through Sept 30, 2012 and is renewable for (4) option years in accordance with FAR Clauses 52.217-8 and 52.217-9. Work will be conducted during normal business hours (8:00AM until 5:00PM). Every effort will be made to ensure minimal disruption to staff and patients. 6.Place of Performance. VA Medical Center, 3710 SW US Veterans Hospital Rd, Portland OR, 97239-2964 7.SCOPE. The contractor shall provide all management, tools, equipment, supplies, and labor necessary to ensure planned and corrective maintenance for all the equipment listed above. Specific Task. 7.1Unlimited Emergency Service. No charges for labor, travel or expenses during normal working hours. Which is defined as Monday thru Friday, 8:00 am - 5:00 pm excluding national holidays 7.2Unlimited Parts. No charge for replacement part which become marginal or defective due to normal use during agreement period 7.3Perform Preventative Maintenance in accordance with manufacturer requirements 7.4Install all authorized product updates and modifications at no additional charge 7.5All work must be done by a certified technician 8.Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).
