Description
The Department of Veterans Affairs, VISN 7, 700 19th Street S, intends to negotiate a sole source Indefinite Quantity/Indefinite Delivery (IDIQ) contract under the authority of FAR 6.302-1, with Biosense Webster, 3333 Diamond Canyon Road, Diamond Bar, CA 91765 to provide the following equipment: DescriptionItem No. EZ STEER DS 8MM BI-DIRECTIONAL CATHETER A26 EZ STEER THERMOCOOL NAV CATHETER K09 LASSO NAV 2515 VARIABLE CATHETER 10P E04 WEBSTER ® DUO-DECAPOLAR CATHETER, WEBSTER ® DUO-DECAPOLAR CATHETER W AUTO ID F05 WEBSTER ® CS BI-DIRECTIONAL CATHETER W AUTO ID H11 COOLFLOW TUBING SET CFT001 CARTO ® 3 SYSTEM REFERENCE PATCH (6-PK) CREFP6 The Birmingham VA Medical Center currently has in use the Cool Flow Biosense Webster Irrigation pump, and requires components that are compatible with the Cool Flow system. Any newly acquired components must be compatible with the existing equipment, software and accessories. As it is impossible to determine the exact quantities that will be required during the contract term, the negotiated agreement will result in an IDIQ contract with a minimum of $77,000 guaranteed and a $100,000 max limit annually. Product delivery is required within three days of vendor ™s receipt of individual orders. This notice of intent is NOT A REQUEST FOR COMPETITIVE PROPOSALS. However, interested parties may identify their experience in providing this equipment by submitting the following information. (1) STATEMENT OF CABABILITY: Submit a brief description [three (3) pages or less] that shows your company can provide the equipment/system. Include past experience in providing the equipment to the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your ability to deliver as stated. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS relative to NAICS 339112, Surgical and Medical Instrument Manufacturing (size standard 500 employees). : (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the Central Contractor Registration (CCR) at http://www.ccr.gov and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in CCR and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.
