Description

HIPAA - BUSINESS AGREEMENT SF- Solicitation, Offer & Award - General Psychiatric Services - SFSU RFP -PSYCHIATRIC SERVICES Transmittal Letter - Psychiatric Services - Santa Fe Service Unit The Albuquerque Area Indian Service has a requirement for Psychiatric services for the behavioral health program within the Santa Fe Indian Hospital, 1700 Cerrillos Road, Santa Fe, New Mexico. Contractor requirements are: Current license to practice psychiatry medicine in any one State of the United States, Board Certified in Psychiatry, documentation of current experience, past performance and education, Services required are for a fulll 8-hour clinic day, 3 times per week, total hours of service per week not to exceed 24 hours. Contract period of performance shall be a period 12 months for the base year, with four, one-year option renewals. Option year renewals will be exercised at the discretion of the Government based on existing requirements, availability of funds and satisfactory contractor performance. All Proposals/Offers received will evaluated on the basis of technical factors, All documentation required by Section M must be submitted at the time of proposal submission. A single requirements type contract will be awarded from this solicatation. Request for proposal No. 12-SOL-242-00007 contains all required forms and information required to submit a proposal. RFP due date is: DECEMBER 23, 2011 - Close of Business: 4:30PM MT
