This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621498, Size Standard 10 Million. The Network Contracting Activity (VISN 2) is seeking potential qualified contractor to provide capitated Community Based Outpatient Clinic (CBOC) in Ulster County New York for the Albany New York Stratton VA Medical Center. Responses to this sources sought must include the following information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged Business, Service Disabled Veteran Owned small business or Veteran owned small business. 3. Does your company have experience with a Department of Veterans affairs capitated CBOC program? 4. What does your company primarily provide? 5. Will your company be a joint venture with another company? 6. How long has your company existed? Responses to this market Survey should be in writing and shall be faxed or emailed to Allen.Johnson2@VA.GOV. No telephone responses will be accepted. Responses must be received no later than 3:00 pm December 9, 2011. No solicitation document is available at this time; this notice is to acquire information only.
