- SOURCES SOUGHT SYNOPSIS REQUEST FOR INFORMATION ONLY THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. THE VA IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. In accordance with FAR 15.201(e), under Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. The Department of Veterans Affairs (VA) is seeking potential sources capable of providing Home Oxygen Equipment Rentals to be delivered to beneficiaries of the VA Butler Healthcare System. Synopsis: 1. VA Butler Healthcare is seeking a source to provide HOME OXYGEN EQUIPMENT RENTAL for our home-bound patients. Services would include but are not limited to providing supplies, materials, equipment, labor, supervision, transportation, patient education, safety management, infection control and recovery of equipment. 2. VA Butler Healthcare currently serves an estimated 300 home-bound oxygen-using patients. 3. All responsive offerors must be able to furnish the oxygen equipment to all the home-bound patients registered with VA Butler Healthcare. Oxygen equipment consists of, but is not limited to: Oxygen concentrators and portable units, back-up systems, oxygen tank carriers, compressors/nebulizers and liquid systems. Delivery devices may consist of, but is not limited to, cannula, face masks, trach mask and oxygen conserving device such as pendant, demand systems and/or mechanical system, disposable humidifiers, tubing connectors and extension tubing and/or hoses sufficient to maintain required/prescribed oxygen flow (not to exceed 50 feet). The contractor will supply 'colored' extension hose/tubing as required to those patients with limited vision to help promote a safe home environment and prevent falls. Equipment and related medical supplies will be provided according to a physician's order. 4. The place of performance includes Eastern Ohio, Pennsylvania, and Northern West Virginia. 5. The NAICS Code associated with this request for information is: 532291 6. Responses to this RFI should be emailed to andrew.taylor3@va.gov not later than January 4, 2010 and should include the following information: a. Name and address of company; b. Capability statement; may include JACHO Accreditation. If not JCAHO accredited, the contractor will have knowledge of and be in compliance with JCAHO standards and must be prepared to provide evidence that it is compliant overall with the JCAHO standards as described in the Comprehensive Accreditation Manual for Home Care re Home Medical Equipment and Clinical Respiratory Services. If accreditation is lost during the contract period, the contractor will immediately notify the Contracting Officer (CO). Subcontractors must follow JCAHO standards and will be monitored by the contractor for compliance. c. Business size; d. GSA/FSS Schedule number. (if available) e. Identification of any socioeconomic categories (SDVOSB, VOSB, 8a, Hub, WOSB, SB) the company falls under f. Any other pertinent information. g. A letter or memorandum indicating a positive intention to provide a proposal if requirement is solicited. The letter should be signed on behalf of your company by a senior procurement official, senior management, owner, or partner (an individual with authority to bind the firm). PLEASE LIMIT RESPONSE TO 10 PAGES 7. Response date (Deadline): Responses must be received no later than 4:00 p.m. on Monday December 12, 2011. Responses received after January 12 2011 may be included with our research but any future consideration for this procurement is not guaranteed if the response is received after the deadline. 8. Veterans First Contracting Program In accordance with IL 049-07-08 "Veterans First Program" the VA, will place SDVOSBs and VOSBs first and second, respectively, in satisfying VA's acquisition requirements when possible. Certain conditions must be met. All VOSBs, including SDVOSBs, must register in the VetBiz.gov Vendor Information Pages (VIP) database, available at www.VetBiz.gov, and CVE certified to be eligible for award of a contract. 9.
