- Standard Form 30 (Amendment 0009) Amendment 0009 was issued to incorporate the following changes; (1) Block 13.A of Section A, the due date of the RFP has extended from 25 Jul 2011 to 21 Dec 2011. (2) Note of Section B has been revised. (3) Section C, YCC001, STATEMENT OF WORK, has been revised. (4) Section G, YCG001, ACCOUNTING AND APPROPRIATION DATA, has been revised. (5) Attachment 2 in Section J, Past Performance Reference List, has been revised (6) Attachment 3 in Section J, Past Performance Questionnaire, has been revised (7) Attachment 4b in Section J, Phasing Schedule Execution Plan Scenario, has been revised. (8) Attachment 5 in Section J, Documents for MACC Seed Project, have been deleted in its entirety. (9) Paragraph (b) of Section L, YCL002, DISCLOSURE OF THE MAGNITUDE OF CONSTRUCTION PROJECTS, has been deleted in its entirety. (10) Paragraph I. of Section L, YCL004, PROPOSAL PREPARATION INSTRUCTIONS, has been revised. (11) Paragraph A.1 and A-2of Section M, YCM001, BASIS FOR CONTRACT AWARD, have been revised. (12) Paragraph A.4 of Section M, YCM001, BASIS FOR CONTRACT AWARD, has been deleted in its entirety. Note: Attachments for this amendment will not be posted on FedBizOpps. The documents will be distributed only to the offerrors who submitted initial proposals by 25 Jul 11.
