- Department of Veterans Affairs (VA) State Veterans Home Program Onsite Surveys Request for Information VA-101-12-I-0026 I. Introduction: This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs Central Office (VACO). VACO is currently conducting market research to locate qualified, experienced, and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. VACO is seeking interested sources (Contractors) for a near-future solicitation and procurement for State Veterans Home Program Onsite Surveys. BACKGROUND Currently, there are 142 state owned and operated state veterans home across the United States and Puerto Rico. The state homes primarily provide nursing home care, domiciliary care and adult day health care. Each of the 142 state facilities must be surveyed annually during the anniversary of their recognition month; annual surveys range from 7 to 18 per month. Recognition surveys, for cause surveys and follow-up surveys are scheduled on an as needed basis and the Contractor will need to make allowances for those. Heavy travel schedules for survey teams will be involved. See attached State Veterans Home Listing. Requirements: VACO requires a Contractor to conduct surveys at state owned and operated veterans homes to monitor compliance with VA standards. The survey team will be comprised of contracted surveyors who will conduct the clinical and life safety portions of the survey and VA Medical Center of Jurisdiction surveyors who will conduct the administrative and fiscal portions of surveys. The Contractor shall conduct annual, recognition, for cause and follow-up surveys for the state veterans ™ home program. The survey process shall apply VA standards utilizing the Centers for Medicare and Medicaid Service process. It is requested that the above information be provided no later than December 7, 2011 at 4:00 p.m. ET. Responses should be emailed to Priscilla Lee at Priscilla.lee@va.gov. Vendors are asked to confirm if they are a Service-Disabled Veterans-Owned Small Business (SDVOSB), Veterans Owned Small Business (VOSB), Women-Owned Small Business or a Small Business. All email correspondence for this project must reference the RFI Number, Project Identifier and Project Title in the subject line of the email.
