- Quotations must be submitted in hardcopy to: Job Corps AQM,USDA Forest Service, ATTN: Donald Watson, 740 Simms Street, Golden, CO 80401. Fax and/or electronic quotations will NOT be accepted. This project is a request for quotations to provide Dental Hygienist Services at the Angell Civilian Conservation Center, located in Yachats, Oregon. Contractors must be registered in the Central Contractor Registration Database located at http://www.ccr.gov, must have completed electronic representations and certifications on the ORCA database located at http://orca.bpn.gov, and must provide a copy of their current dental hygienist license to practice in the state of Oregon with their quote. All other requirements are included in the solicitation, which shall be posted on the FEDBIZOPPS website for downloading. The performance period is 365 calendar days, (April 1, 2012 through March 31, 2013), with 4 option year periods, which may be exercised at the Contracting Officer's discretion. The NAICS code is 621210. The small business size standard is $7 million. This project is 100% Set-aside for Small Business. Paper copies of the solicitation will not be issued and telephone requests or fax requests for the solicitation will not be accepted. All responsible sources may submit a quote which will be considered. Quotes can be submitted directly from the information provided in the solicitation package as posted. All questions related to this solicitation must be submitted in writing to Donald R. Watson at drwatson02@fs.fed.us. Only written questions will be addressed.
