- Contractor shall provide professional and medical related clinics as specified herein. Clinics shall be performed at the Rochester Outpatient Clinic (ROPC), 465 Westfall Road, Rochester, NY and/or Canandaigua VA Medical Center (VAMC), 400 Fort Hill Avenue, Canandaigua, NY 14424 in accordance with the terms, conditions, provisions, specifications and schedules herein. In addition Contractor shall provide professional and related medical services as specified herein. Work shall be performed at the Contractor ™s facility in accordance with the terms, conditions, provisions, specifications and schedules herein. Procedures shall include, ETT, Holter Monitors, Echocardiograms and Nuclear Studies. Contract period of performance to commence on March 1, 2012.
