- Description
- VA North Texas Health Care System has a requirement for in-home Respite Care services to chronically ill or disabled Veteran beneficiaries. The Contractor shall provide homemaker/home health aide services on a short-term basis to allow the beneficiary's caregiver a period of relief from the demands of daily care for the beneficiary. Services shall be provided in the following counties: Archer, Baylor, Bryan, Camp, Choctaw, Clay, Collin, Cooke, Dallas, Delta, Denton, Eastland, Ellis, Fannin, Franklin, Grayson, Henderson, Hood, Hopkins, Hunt, Lamar, Jack, Johnson, Kaufman, Montague, Palo Pinto, Parker, Rains, Red River, Rockwall, Smith, Stephens, Tarrant, Throckmorton, Titus, Van Zandt, Wise, Wood, and Young (NOTE: Bryan and Choctaw counties are located in the State of Oklahoma.) Services shall be provided as required during the hours/days of 7:00 a.m. to 6:00 p.m./Monday through Friday, including holidays. The Contractor shall possess current applicable state license & liability insurance and shall maintain annual staff training & proficiency. The period of performance consists of a base period beginning February 1, 2012 and ending January 31, 2013 with four (4) one year option periods. The NAICS code is 621610 and the SBA size standard is $13,500,000. The estimated total annual quantity, based on current case load, is 432 hours. This solicitation is 100% set-aside for Small Business. The anticipated solicitation release date is on or about December 9, 2011 to which responses will be due no later than Noon on January 6, 2012.
