- The purpose of this amendment is to correct or include the following: 1. Number 24 of solicitation is changed to reflect the revised offer due date: Offers are due no later than 4:00pm on 12 December 2011. Electronic submissions via fax will not be accepted. Offers shall be emailed. Proposals can be emailed to Marvinia Adams, Contract Specialist, at marvinia.j.adams.civ@mail.mil. Proposals not received by the closing date/time may not be accepted and/or evaluated. Acknowledgement of Amendments 01, 02, and 03, are required. 2. Attachment 1 PWS has been updated to include additional information to specific tasks section 5.1, update period of performance (section 1.5), update technical exhibit 1 - place of performance, and add technical exhibit 4 - historical data. 3. Attachment 3 Line Items has been changed: - added and modified line items and line item descriptions - removed from line items 0002, 0005, and 0008 the quantity of 7000, and unit of issue of U.S. Dollars. - included a pricing table that shall be used to provide the Government pricing for Line Items X002 and X003. 4. Attachment 5 Proposal Format has the following changes: - Proposals shall be submitted prior to the closing date and time identified in accordance with FAR 52.212-1 to marvinia.j.adams.civ@mail.mil. - Submit signed and dated proposals to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers must be submitted via email. Pricing shall be submitted on the attached pricing schedule (attachment 3). It shall contain the pricing schedule table, contractor's unit price, and total price for the established Contract Line Item Numbers (CLINS), base year, and all option years. - Offerer's proposal shall consist of: Volume I - Company Information/Business Class Size, Attachment 2 - Continuation of Block 17, and Attachment 7 - IF NOT registered in ORCA Volume II - Technical Proposal Volume III - Price Proposal - Attachment 3 - The page limit requirement for volume III has increased to 10 - Page Limit does not include attachment 7, covers for volumes, tables of contents, glossary of abbreviations and acronyms, indices, title pages, cross reference indices, and section dividers/tables if they are inserted solely to provide ease to the reader in locating parts/sections of the proposal. They will be counted if they contain any other information, i.e., diagrams, extraneous data, etc. Pages marked "This page intentionally left blank" will not be counted. 5. Attachment 6 Evaluation - added "(attachment 3)" within the text at (b)3a: it now reads, Price Proposal: the offeror shall submit an original (PDF format) and one copy (offerors preferred format) of its price proposal. The Offeror's price proposal (at a minimum) shall consist of a completed Section B (ATTACHMENT 3). The offeror shall propose a firm-fixed-price for this effort. In addition, any exceptions to this solicitation should be submitted with the price proposal. 6. Attachment 8 Questions and Answers has been attached. Any questions submitted after this modification may not be answered. 7. All attachments will be uploaded with this amendment. Contractor shall submit proposal based on the most recent amendment and attachments. Contractor shall acknowledge all amendments to this solicitation. The solicitation response date has not changed and all offers must be received no later than 4:00pm on 12 December 2011.
