- Exhibit A, CDRL Attachment 1, PWS - Revision 07 Dec 2011 Standard Form 30, Amendment of Solicitation Amendment 0002, RFP This amendment updates the PWS (Chapter 1, Description of Services) and add Contract Data Requirements List. Please note: Revised proposals are not required at this time. Any proposal changes in response to this amendment should be included in the Final Proposal Revision. ***************************************************** The TEP Matrix MS Excel Spreadsheet dated 8 Jun 2011 and included with the Request for Proposal (RFP) contained an error. On the "Total Contract" worksheet, the TOTAL COST FOR ALL CLINS cell (G37) omitted the Support of Personnel and Contractor Operations at FMSC at Fayid Air Base - CLIN 0007 (1 June 2012 - 30 November 2012) cell (G23) in the formula. The correct formula in cell G37 is as follows: =SUM(G35,G32,G29,G23,G26,G20,G17,G14,G11,G8,G5) An updated version of the TEP Matrix MS Excel Spreadsheet is now included as a file on the EPSSIVFA863011R5002 FBO webpage. Any proposals received that do not have the correct formula in cell G37 will be corrected during proposal evaluation. The calculated price of CLIN 0501 is used for evaluation purposes only. Refer to the TEP Matrix instructions for information regarding this calculation. No input is required in Section B for CLIN 0501 or associated subCLINs. Section L, Page 11, Para 7.0 The heading "7.0 Volume IV - Contract Documentation" should read "7.0 Volume V - Contract Documentation" Clarification: The Response to the Program Management Exercise Scenario should be provided by the offeror in Volume II. A reference to this response may be made by the offeror in Volume V. *********************************************************** The following information is provided for clarification: Section L, 4.2.5, Number 1 refers to PWS Chapter III, Section 10.1. The correct reference is PWS Chapter IV, Section 10.1. The page limit for Volume III, Relevant Past Performance is limited to the number of pages generated by the PPI database tool described in Attch L-3. Organizational Structure Change History should be considered among the documents excluded from page limitations. Vehicles with diplomatic license plates included on the PFP listing are indicated by a "/" in the license plate number. All other listed vehicles have customs license plates. The joint inventory of all PFP shall begin NLT 30 calendar days after award of the contract. Workload estimates in Appendix C of the PWS are based on historical information and do not supersede PWS requirements for Security, Fire Protection or other performance areas. After receipt of proposals, a Past/Present Performance Questionnaire will be distributed by the USG to Customer Points of Contact. This Questionnaire is included as a file on the EPSSIVFA863011R5002 FBO webpage. The RFP will be amended to specify the Questionnaire will be used as a source of information during Past Performance evaluation. *************************************************************** REVISION: An error was discovered and corrections have been made to Appendix AJ (attached). An updated PWS incorporating changes to Appendix C and Appendix AJ is also attached. All changes are in red. Note: The proposal due date has not changed and are due on 10 Aug 2011, 4:30 p.m. Eastern. ************************************ 1. Past Performance Information (PPI) Template database to be useded when submitting PPI in accordance with Section L. 2. An update to Appendix C of the Personnel Support Services (PSS) Performance Work Statement (PWS) has been posted. Changes are indicated in red text. This update corrects the calculated total of multiple workload estimates and incorporates other administrative changes. This update does not include any new or additional workload estimates. 3. List of Attendees, Pre-Proposal Conference (19-23 Jun 2011). 4. Responses for questions that were asked during the Pre-Proposal Conference. 5. Contact information for LandLords. NOTE: Multiple contractors have submitted questions related to PWS requirements, contract type or CLIN structure. Answers will not be posted in response to these questions. A draft RFP was posted on 25 March 2011. Questions and responses to the draft RFP were posted on 12May 2011. This information is accessible under the presolicitation notice. Multiple contractors have submitted questions related to the economic situation in Egypt. It is the sole responsibility of each offeror to research the economic situation in Egypt when developing proposals. *********************************************************************** SUBJECT: FA8630-11-R-5002, Request for Proposal (RFP), Egyptian Personnel Support Services IV. Contractors are requested to provide a complete, fully substantiated proposal, in accordance with the attached Performance Work Statement, the model contract, and Section L with all stated requirements. A Firm-Fixed Price (FFP) contract is anticipated. You are cautioned that until a contract is executed any effort performed in support of the anticipated effort is performed strictly at your own risk and expense.
