- RFQ bid sheet Wage Determination Performance Work Statement/Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and simplified acuisition in FAR 13.1, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) # F4D4A1135A001 Submit written quotes- quote template provided- via email. Oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) (www.ccr.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-54 and DFARS Change Notice 20111207. This procurement is being issued as a 100% small business set aside under North American Industrial Classification Standard (NAICS) 812320 and Size Standard $5 million. Project Number: F4D4A11210A001 Project Title: Clinical Laundry 1. Quote shall conform to the CLIN structure as established in this synopsis/solicitation via the attached RFQ bid sheet. 2. Quotes shall be valid for 30 days after close of solicitation. 3. Please see attached Statement of Work for detailed description of required duties. 4. Performance shall be at Vandenberg Air Force Base, CA, shipping is Free-on-Board(FOB) Destination 5. Period of performance: Base Year:, 01 January 2012-31 December 2012 Option Year 1, 01 January 2013-31 December 2013 Option Year 2, 01 January 2014-31 December 2014 Option Year 3, 01 January 2015-31 December 2015 Option Years 4,: 01 January 2016-31 December 2016 Optional six month extension: 1 January 2016-30 June 2016 6. All questions or comments must be provided to the Contract Specialist in writing; telephone and other means of oral communication will not be permitted. The Contract Specialist is John P.
