- DEPARTMENT OF VETERAN AFFAIRS SOUTHEAST LOUISIANA VETERANS HEALTH CARE SYSTEM P.O.BOX 61011 New Orleans LA 70161-1011 Notice Date: 12/13/2011 Set Aside: N/A Notice Type:Special Notice IAW: F.A.R 6.303-2 This is not a request for proposal or a request for quote. There is no solicitation documentation available Under F.A.R Part 6.303-2. This is a notice of intent. The Southeast Louisiana Veterans Health Care System intends to negotiate a sole source firm fixed price contract with BRIAN MACOBIN, located at 7731 Panola Street New Orleans Louisiana 70118-4240. The services are to include conducting manualized prolonged exposure therapy sessions via telemedicine, and iPhone4 with rurally-located veterans suffering from Post Traumatic Stress Disorder (PTSD) in accordance with the approved MIRECC funded study, and mandatory requirements. A Sole Source Justification and a Statement of Work have been prepared in support of this acquisition, and are available upon request. Oral communications are not acceptable in response to this notice. Interested parties may identify their interest and capability to respond to this requirement no later than December 22, 2011 @ 4:00 PM CST offers submitted past the appointed time shall be considered a no bid. All information submitted should support the offers capability to provide services required in the Statement of Work, and shall be provided at no cost to the Government. Responses must be in writing and must provide clear and concise documentation, indicating an offerors ability to meet the services required. Responses will be emailed to Gary.Baker3@va.gov verbal responses are not acceptable and will not be considered. Emails are not to be in the excess of 5MB. Prospective offerors shall complete electronic annual representations and certification at http://orca.bpn.gov, shall be registered in the Central Contractor Registration (CCR) database at http://www.ccr.gov prior to award of the contract.
