Description
Notice of Intent for Sole Source for Nursing Home Services. The Veterans Affairs James J. Peters Medical Center, Network Contracting Acquisitions (VISN3) intends to enter into sole source procurement Pursuant to FAR Subpart 6.302-1, with San Simeon by the Sound Center for Nursing and Rehabilitation, Inc. San Simeon is located at 61700 Rte 48 at Greenport, NY 11944-2206. The period of performance will be for Fiscal Year 2012. Services will be performed for James J. Peters Medical Center (Northport, New York). VAMC Northport covers a large catchment area, which includes both Nassau and Suffolk counties, which stretches from the eastern edge of NYC to Montauk Point, which is approximately 100 miles long. San Simeon nursing facility is more than 40 miles from the nearest other contract nursing home, and will provide service to veterans on the eastern end of Long Island who were previously not served well by the CNH program. In addition, this new contract facility will support the new VAMC Northport CBOC in Riverhead and the Home Based Primary Care team, which will work with frail elderly homebound veterans with caregivers who may eventually need nursing home placement. The families of veterans placed in nursing facilities have consistently requested facilities as close as possible to their place of residence. The CNH program has tried as much as possible to meet these requests, as it encourages family visits, which enhances quality of life for veterans placed in nursing facilities. The intent is for nursing home services to be provided at the VA James J. Peters Medical Center in Northport, New York. Any firm that believes it can provide the stated service mentioned, please provided the supporting documents demonstrating their equipment/ capability; and specifications no later than December 19, 2011 @ 9:00am. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures.
