- 12/16/2011THIS IS A COMBINED SYNOPSIS AND SOLICITATION REQUEST FOR PROPOSAL. The NETWORK CONTRACTING OFFICE 19, Rocky Mountain Network, 4100 E. Mississippi Ave, Glendale CO is soliciting for the Veterans Administration certified Service Disabled Veteran Owned Small Business (SDVOSB) for the Salt Lake City VAMC, SLC, Utah. Please refer to the Performance Work Statement (PWS) attached to the solicitation. Solicitation shall be posted on 12/16/2011 for approximately three (3) weeks; with a response due date of 01/06/2012. SCOPE: Contractor shall provide (1.0 FTE) Histopathology Technician. The contractor shall provide a clinical laboratory professional that will work closely with the pathologists in advanced technical and limited supervisory work for the preparation of body tissue specimens for microscopic pathological examination. The contractor is required to work under strict deadlines and must be able to work efficiently with fragile equipment such as glass slides and/or small knives. The work involves responsibility for accuracy in preparing, cutting, staining, and mounting tissue sections to assist pathologists in making correct diagnosis of patient's pathologic disease and/or cause of death. Work is performed in accordance with standard laboratory practices and procedures and technical guidance is available for unusual cases. Work is performed under the supervision of the laboratory manager who reviews policies and procedures and pathologists who review and examine prepared slides for microscopic pathological examination. Report to Terry Gallardo, 801-582-1565 x 1467 Actual performance shall be provided Monday through Friday 0700-1530 (40 hrs/wk) for a period of six (6) months April 01, 2012 through September 30, 2012, excluding federally recognized holidays. Solicitation VA-259-12-RP-0027 Histopathology Technician Non Personal Services (Professional Services) has been assigned and should be referenced on all correspondence regarding the solicitation. The NAICS Code 621511 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of $13.5 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract.
